MEMORANDUM OF UNDERSTANDING

KNOW ALL MEN BY THESE PRESENT:

This agreement entered into by the Tbolok Elementary School and Barangay
Rural Health Unit to conduct Rapid Antigen Test and RT-PCR Test in case there is a suspected/ positive
Covid-19 leaner/s for testing and confirmation.

This Memorandum of Understanding takes effect upon the signing of these

Memorandum of Understanding.

IN WITNESS WHEREFORE, the parties have hereunto affixed their signatures in

this Memorandum of Understanding this March 31, 2022.
