JOINT AFFIDAVIT OF ARREST

We, the UNDERSIGNED Police Officers, both members of the


Philippine National Police (PNP), presently assigned in Tuguegarao City,
Cagayan, do hereby depose and state THAT:

1. An OPLAN BUGAW Team was formed by the Inter-Agency Council


Against Trafficking (IACAT). The team aims to conduct surveillance
operations in bars, night clubs, and other establishments for possible
entrapment of violators of the provisions of Republic Act No. 9208,
otherwise known as the  "Anti-Trafficking in Persons Act of
2003", defined and penalized under the laws of the Philippines;

2. At about 9 o’clock in the evening of February 6, 2020, Police Officers


Andres Melad and James Bonifacio conducted surveillance around
the area of Macapagal Avenue, Balzain East, Tuguegarao City and
found that men go to a certain Moonlight Bar not only to eat and
drink, but also to get women prostitutes;

3. At around 10 o’clock in the evening of March 05, 2020, after the


above-mentioned surveillance, we, the undersigned, together with
Andres Melad and James Bonifacio became members of the team
who conducted an Anti-Human Trafficking Entrapment and Rescue
Operation, where the two aforementioned policemen posed as
customers at the said Moonlight Bar located along Macapagal
Avenue, Balzain East, Tuguegarao City, Cagayan Valley, and avail of
their services;

4. The respondent is the General Manager/ Owner of Moonlight Bar, a


business engaged in food, entertainment and guest relations
services located along Macapagal Avenue, Balzain East, Tuguegarao
City, Cagayan Valley;

5. Prior to the entrapment operation, we agreed to wait for the go


signal of the two decoys through a miss call before we enter the
premises of the said establishment to arrest the suspect/s and
rescue the victims;
6. Upon receiving a miss call from the two decoys at around 11:15 PM,
we immediately barged in the Moonlight Bar and saw Grace talking
to our decoys while holding the marked money;

7. Seized and recovered in the possession and control of the arrested


person were the following, to wit:

a.) Marked Money, (state the denomination and serial


numbers);
b.) (Name of documents
promised); d.) Other
evidence confiscated;

8. Suspect together with the seized and recovered evidence was


brought to the Tuguegarao City Police Station for booking and
proper disposition. Likewise, the arrested person was apprised of
their rights as provided for under the Constitution of the
Philippines.

9. The report of the PNP Crime Laboratory Group Physical


Identification Report No. 6665 dated March 5, 2020, revealed that
suspect GRACE TENDENILLA was POSITIVE for the presence of
Bright Yellow Ultraviolet Fluorescent powder on both palm and right
dorsal portion of the hands.

WE are executing this Affidavit to attest to the truth and veracity of


the foregoing facts and events, and for the purpose of instituting a
criminal case against respondent GRACE TENDENILLA for violation of
Republic Act No. 9208, otherwise known as the  "Anti-Trafficking in
Persons Act of 2003".
