Sub: Minutes of Fire Mock drill

Incident happened at 10.30 am

Clearance given at 11.04 am (Total time:0.34 hrs.

In internal safety team conducted the mock drill before that awareness training
given as pre mock drill for all the ERT member others.

 Sight controller : Mr.P.Digamber

 Incident controller:Mr.M.C.Roy
 Emergency coordinators:
Mr.Prasana,Mr.K.Srinivas , Mr.Peddi Raju & Mr.Anjaiah.
 Evacuation team:Mr.Venkat rao,Mr.K.Gopal, , Mr.Ali
 Fire fighting team: Mr.Kutumba Rao,Mr.Govind & Mr. Viay
 Organization team (Liason Officer):Mr.K.Ramesh Babu.

 Fire incident (Wood & solvent fire) happened in ETP area at 10.30 am. it
was seen by Supervisor and informed to incident controller at 10.31 am
 Immediately incident controller informed to Sight controller at 10.31 am.
 Sight controller advised security to alert all the employees in the factory at
10.32 am, on siren given three times, all the people gathered at assembling
point at 10.33 am.
 Sight controller given fire information to : 1.Fire service dept.
2. Revenue dept. at10.31 am
3. Factory dept.
4. PCB dept.

 As per the advise of Sight controller heads counted by Liaison officer at

10.40 am.

 Incident controller requested to Sight controller to send the Evacuation tea

for clearing the drums at incident spot at 10.32 am, then sight controller
sent the evacuation team.

 And also send the Fir fighting team at 10.33am.

 2 Fire billiards’ reached to spot at 10.40 am, and also Deputy General of
Police (Fire dept) reached the spot at 10.45 am sends to incident spot in
coordination with Emergency coordinator.
 After that Sight controller advised to emergency coordinator to check inside
Production blocks (if any employees was risked) at 10.40 hrs. At 10.45
am,he reported that no one was risked inside area.

 Sight controller received call from incident controller for foam gun purpose at
10.34 am, immediately send fire gun.

 At this time alerted visitor’s vehicle drivers to move their vehicles out side
from inside factory at 10.45 am.

 By conformation purpose again sight controller send the organization(liaison

officer) team for checking in side plant at 10.45 am, he reported that no one
Was risked inside plant at 10.53 am.

 Sight controller enquired to incident controller about the wind direction, he

says i.e. west to south at 10.55 am.

 Incident controller requested to Sight controller to send the first aid team at
10.58 am , then send the structure and medical treatment given at safety
control centre at 11.00 am.

 Sight controller informed to incident controller to move all the people from
incident spot to assembling area at 4.01 pm. All the employees were
assembled at 11.02 am.

 As per the report of emergency coordinators and liaison officer report Sight
Controller given clearance for all the employees to moved their respective
Working areas at 11.04 pm.
