PAKYAW AGREEMENT

KNOWN BY ALL MEN BY THESE PRESENT:

THIS AGREEMENT, made and entered into this 6th day of JANUARY, 2018 at San
Jose, Occidental Mindoro by and between the Department of Education, Division of
Occidental Mindoro represented herewith by the Principal referred hereafter to as the
IMPLEMENTING AGENCY and the PAKYAW GROUP represented by their Group Leader
ZALDY COLITA herewith referred to as the PAKYAW CONTRACTOR.

WITHNESSETH

That for and in consideration of the hereunder conditions, the PARTIES hereto covenant
and agree as follows:

1. That the PAKYAW CONTRACTOR shall provide labor necessary for the proper and
faithful performance of the work described at the Program of Work and attached to
this amount of THREE HUNDRED PESOS (P300.00).

2. That the PAKYAW CONTRACTOR shall complete the work in accordance with the
specifications, under the supervision of the Principal or designated person appointed
for the purpose by the IMPLEMENTING AGENCY.

3. That the PAKYAW CONTRACTOR shall comply with all the terms and conditions of
this contract more specifically those contained in the program of work or other
essential documents, which are hereby made and acknowledged as an integral part
of this agreement.

4. That the PAKYAW CONTRACTOR agrees to commence work within two (2) days
upon signing of this agreement and to complete the same within 15 calendar days,
Sundays and legal holidays excluded;

5. That the Principal shall submit to the Schools Division Superintendent Office periodic
report on the time records of the workers, the group accomplishments and
corresponding payment hereof.

IN WTNESS HEREOF, the PARTIES aforesaid have hereunto affixed their signature on the date
and place above written.
