COMMUNITY PARTNERSHIP AGREEMENT

between:

DON BENITO AGRO-INDUSTRIAL HIGH SCHOOL

and

PHILIPPINE NATIONAL POLICE – POZORRUBIO

I. TITLE.

This project was conducted in observance of the “Whole Nation Approach” entitled the DUTERTE
LEGACY BARANGAYANIHAN CARAVAN TOWARDS NATIONAL RECOVERY.

II. PURPOSE & SCOPE

The purpose of this agreement is to clearly identify the roles and responsibilities of each party as they
relate to providing people access to the basic services from the government. Also, this caravan was a
collaboration of the different private sectors, volunteers and government agencies including the BFP, Rural
Health Unit, MSWD, PSA PhilSys, Faith Based and Barangay Based Advocacy Group of BADAC
members from DONMABACA community.

A. Activities
1. Free Police Clearance 7. Distribution of School Supplies
2. Free Check-up and Medicine 8. Distribution of Hygiene Kits
3. Issuance of National ID 9. And other related activities and services.
4. Lectures
5. Feeding Program
6. Distribution of Flyers

III. TERM OF PARTNERSHIP

The partnership shall commence on November 12, 2021, exactly 8;00AM – 5:00PM.

IV. PLACE OF PARTNERSHIP

The venue for the said activity/ caravan will be held at Don Benito Agro-Industrial High School
Gymnasium.
