INTRODUCTION:
This Performance Appraisal is an essential part of the Management process for the effective utilization of Human
Resources. It consists of the process of reviewing the employee's performance / progression in a job within the
prescribed period.
It also consists of the formal review which will consider performance in the job as a whole using the major job
objectives and summary of actual achievement, to highlight the aspects of the job that were most significant within
the period.
This process seeks to embody the Company's commitment to regular measurement of effective performance by
staff as part of their orientation towards result.

This Performance Appraisal Form provides an opportunity for discussion on strengths and development needs.
Assess how skills have been applied in satisfying job requirements. Check appropriate box. Assessment must be
supported by specific comments.

REMARKS:
NOTE: Immediate Superior / Department Head must conduct an appraisal interview with the employee not later than one

how the employee can improve further on his/her performance.
