1.0. INTRODUCTION
This Project Emergency Response – Plan (ERP) describes the process for responding to a crisis
that may arise during the construction and commissioning of Project -Supply and Installation,
Sem-Calaca Power Corporation (SCPC) Rehabilitation Works: Electrical System Upgrade.

The ERP is based on the organizational and management principles of the ABB Way Emergency
Preparedness and Response Plan SA-M-10. This system provides a globally accepted standard
management structure for incident and emergency management. This system further
establishes consistent principles, terminology, and procedures to be used by both ABB and ABB
Contractor in the management of emergency incidents during the construction and
commissioning stage of the SCPC-Electrical System Upgrade project.

1.1. PURPOSE

The purpose of this document is to outline the emergencies that are the responsibility of the
Contractor (ABB) and to detail the necessary emergency response measures, equipment and
facilities that need to be planned, implemented and maintained during the construction and
commissioning of the “SCPC-Electrical System Upgrade”. This document also addresses the
recovery stage to enable recovery from an emergency incident. In detail, the objectives of this
Emergency Response Plan are to:

(1) Identify the nature and type(s) of emergencies that may arise during the
construction and commissioning phase of the project that ABB will be responsible
to manage.

(2) Identify the required equipment and facility, personnel and training at project site
necessary to manage the potential emergencies and incidents that have been
identified, analysed and assessed as per SA-M-02 “Hazard identification and
assessment of risks and opportunities”.

(3) Establish the roles and responsibilities of Contractor and Subcontractor staff on
site during an incident or emergency;

(4) Ensure that each emergency is managed in accordance with a detailed set of
procedures during the construction stage, up to mechanical completion, of the
project; and

(5) Ensure that all Emergency Response activities are undertaken in accordance with
Customer Emergency Response Procedure for construction and commissioning.
1.2. SCOPE
This ERP is a contractual requirement and is intended to be binding on all members of the ABB
Project Management Team as well as all ABB staff and Sub-contractors. This ERP enables ABB to
take actions necessary to protect project personnel and the public, the environment, and
assets.

During construction and commissioning of the SCPC-Electrical System Upgrade Project this ERP
will be used by Supervisors and Subcontractor’s personnel who have a duty of care in the event
of any emergency, to protect and assist all members of the workforce and to minimize any
adverse impacts of any emergency incident on workers, the general public and the environment.
This ERP covers all construction sites, including access roads and worker camp(s).

2.0. REFERENCES

2.1. DEFINITION

2.1.1. Incident

An incident is any event that causes or has the potential to cause (Near Miss) personal injury,
damage/loss of assets, environmental damage or harm to the health and/or safety of any
individual.

Incidents do not include operations, maintenance, quality or reliability issues which had no HSE
consequence or potential. Incidents do not include degradation or failure of plant or equipment
resulting solely from normal wear and tear unless this results in injury to persons, damage to the
environment, asset loss or reputation damage.

2.1.2. Crisis

A crisis is an incident (either a situation or an event) that has the potential to impact the wider
organization in terms of safety of life, continuity of business or damage to reputation; where
there is a degree of complexity so that it is unclear what action should be taken; and where
extraordinary resources are required to bring the incident under control and manage it to a
satisfactory outcome.

2.1.3. Emergency

An emergency is an event or situation that results in an abnormal state and requires an


immediate response in order to contain it. Typical emergencies that could occur during
construction and commissioning include:

1. Work site accidents including severe injuries and fatalities;

2. Medevac emergencies;

3. Medical (illness and injury);

4. Electrocution contact with electricity/3rd degree burns.

5. Facility Emergencies (fire, explosion, toxic gas leakage, hazardous material spillage etc.);

6. Road Traffic Accident (RTA) / Motor Vehicle Crash (MVC);

7. Chemical or fuel spillage;

8. Fire in workers camps ; and/or

3.0. ON SITE EMERGENCY RESPONSE FRAMEWORK


ABB will maintain a tactical response capability to manage all local site incidents and
emergencies that it is responsible for appendix A,B,C,D,E. This will involve the formation of one
or more Emergency Response Teams (ERTs) trained and equipped to deal with the likely
emergency incidents and the designation of a trained and experienced On Scene Commander
(OSC) who will be responsible for managing the operational components of each emergency
incident.

3.1. Emergency Response Team(s)

Throughout the project construction and commissioning stage the HSE Supervisor will develop
Emergency Response Team(s) (ERTs) that consist of experienced, trained personnel, able to deal
with emergencies that may arise at the workplace and camp.
All emergency responders will be fully responsible for controlling the emergency scenarios and
notify the senior management and customer ERT team for immediate response. The Emergency
Response Team(s) shall be involved in training programs and drills (to be established by the
customer ERT or HSE) which shall include, but not be limited to the following:

(1) Firefighting training (4) Fire prevention training

(2) Evacuation procedures (5) Vehicle incident

(3) First Aid and Rescue (6) Gas/Steam Release

Each ERT team will be led by a team leader responsible for the co-ordination of the team’s
activities.

3.2. On Scene Commander

The On Scene Commander (OSC) is the person designated to coordinate the rescue efforts at
the rescue site and control the emergency responders. He will be supported by the ERTs along
with ABB’s HSE and Senior Site management personnel. He will also be supported as required by
SCPC’s ERT team.

3.3. Incident Control Centre

SCPC HSE Department/Incident Command Centre is the focal point in all major emergencies at

4.0. RESPONSIBILITIES / EMERGENCY RESPONSE COORDINATOR


The following roles and responsibilities have been established for the key personnel involved in
the management of emergency incidents during the construction of the SCPC project.

4.1. Site Manager


Site manager is the person who is responsible for the overall preparedness of the contractor to
respond to all emergency incidents. The site manager shall have the following emergency
response responsibilities:
1) Ensuring adequate resources and trained personnel, medical and other emergency
facilities needed to implement and execute the ERP are available during the construction
and commissioning period.
2) Ensure discipline is maintained at all locations during an emergency.
3) Participate in the scheduling and enactment of emergency response drills, exercises and
training set by the customer HSE Department/ERT.
4) Monitor the arrangements through the Hub HSE Manager or HSE Supervisor to ensure
that the emergency response procedures and their implementation are appropriate for
the needs of the project.
5) Ensure that after an emergency and before routine activities continue all managers and
line supervisors shall assess facilities for damage and loss and take measures to rectify
any unsafe conditions.
6) Liaise with Customer during actual incidents and emergencies and appoint
appropriate/trained admin, HSE and technical staff to join Customer Incident
Management Team in the event it is mobilized.
7) Ensure that each emergency incident is documented, investigated, reported and that all
necessary actions identified are closed out.

4.2. HUB HSE Manager


Hub HSE Manager has the overall responsibility for all HSE issues on site. The Hub HSE Manager
is also responsible for the development and implementation of HSE plans and procedures. The
Hub HSE Manager shall have the following emergency response responsibilities:
1) Identify plausible emergency response scenarios using Customer Emergency
Preparedness and Response procedure. All emergency risks to be captured and tracked in
a risk register.
2) Develop, implement and maintain management plans and procedures to manage each
risk scenario identified. These plans will include procedures to guide team members from
initiation/mobilization through to response and recovery.
3) Ensure a documented process is in place and operational for accounting/tracking all
personnel during and after emergency scenarios identified.
4) Ensure that regular exercises are undertaken to practice and test the robustness of
emergency response plans (including consideration of external communications and
involvement).

5) Ensure, in conjunction with site management, that resources data of personnel,


equipment’s, materials, etc. are available on site
6) Initiate and regularly review/update the ERP.
7) Set up drills and exercises in coordination with Customer’s ERT & HSE Department
8) Undertaking lessons learnt and debriefing exercises after all exercises and emergency
events to ensure all lessons are identified and captured

4.3. Site HSE Supervisor


The Site HSE Supervisor shall have the following emergency response responsibilities:
1) Ensure that trained personnel, equipment and facilities needed to respond to emergency
situations are defined, identified, available and periodically tested.
2) All muster points are designated and maintained throughout site
3) Ensure all emergency response equipment is well maintained, serviced and fit for
purpose.
4) Ensure the establishment and maintenance of contact details of call out personnel and
emergency services and agencies (refer to Appendix F)
5) Ensure the emergency response teams are established and trained.
6) Ensure information and communication to staff through distribution of the ERP and any
documents or updates following any emergency response exercise or after an actual
incident by incorporating findings

4.4. On Scene Commander (OSC)

The On-Scene Commander shall have the following emergency response responsibilities:

4.5. Emergency Response Teams (ERTs)

1) The team(s) shall be responsible for recovering any casualties and properly transporting
them to the Medical Center.

2) The team shall also assist the nurse and first aider on duty in rendering medical aid to
injured parties. If nursing staff and first aider are not available, the leader of the team
shall take charge by administering first aid and sending injured or sick person(s) for
further treatment to the medical center or local accident and emergency hospital.

4.6. First Responders


Anyone spotted any type/kind of emergency, the first action is as follows:
• First make sure it is not life threatening.
• If so, call for help by means of Mobile to the nearest Rescue Center.
• Do not endanger yourself by helping the others.
• Never go away from the scene until the help to arrive.
• Upon arrival of the ERT if in need of help do so.
• Not sure what to do or to help; do not do it.

• If required for more help summon them by means of the easiest communication
way.

5.0. PRE-EVENT PLANNING


Any emergency occurring on construction sites has serious consequences. People may be
injured or fatalities may occur, buildings/structures and equipment can be damaged or
destroyed. Such situations demand adequate response, with rescue and relief measures to
handle such events safely, quickly and efficiently.it is therefore necessary to ensure that all
persons on site are aware of their responsibilities during an emergency response and duties of
key personnel must be established.as noted above the HSE Manager is responsible for all site
pre-event planning

5.1. POTENTIAL EMERGENCY SCENARIOS


This section identifies the potential emergency scenarios that may affect the SCPC Project
during construction including:
• Fatality / Serious Injuries (Fall from height / fall of load resulted to serious physical
injury)
• Electrocution/Contact with electricity/3rd degree burns.
• Fire and Explosion
• Motor Vehicle Accidents (MVA) or Personal Injury
• Earthquake
• Severe Weather/Storm (Typhoon)
• Underground Cable Strike

6.0. TRAINING AND DRILLS


Exercises on site shall be conducted at least quarterly (depending the schedule of the customer
drills) to test the site’s emergency response capabilities across the range of expected or likely
emergency incidents. In particular:
1) Hub HSE manager and Site HSE Supervisor will identify and provide or obtain,
appropriate emergency response training for all employees deemed necessary for
managing emergencies that are likely to affect the site operations.
2) Supervisors – will be responsible for ensuring that they and the employees who report to
them, attend emergency response training appropriate to his/her area of work
responsibility.
3) All Employees – will be responsible to attend emergency response training appropriate
to his/her area of work responsibility.
4) Emergency Responders – all emergency responders will be selected on a volunteer basis,
will be qualified physically fit to perform required duties, and will be responsible to
attend appropriate training necessary and to respond to emergencies that are likely to
affect the site.
Reports of each training exercise will be prepared and forwarded to the SCPC. Any remedial
actions required shall be identified. The generic emergency procedures shall be updated if
necessary following each emergency drill or actual incident. Training and information will be
clearly communicated via notices in multiple languages for all persons working on site.

Training shall be conducted on a regular basis for the team involved in the emergency
procedure.
A comprehensive emergency response training schedule will be prepared and updated twice
yearly.

7.0. RESPONSE TO AN EMERGENCY

7.1. Raising the Alarm and Communication


In the event of any emergency workers will raise the alarm by informing their supervisor, who in
turn will notify the HSE department who in turn will notify site management and the OSC. It is
the OSC’s responsibility to contact SCPC and the HSE manager’s responsibility to initiate the
response activities as detailed below.
On hearing the alarm, all personnel shall vacate the room or work place. If it is safe and prudent
to do so, before leaving the working place the individuals concerned shall leave the work place as
secure as possible to avoid further damage or injury e.g. Switching-off of welding machines,
closing gas cylinders, turning off running machines, gas burners, any electrical light or
appliances in their rooms. All personnel shall assemble at the muster point according to the
color code of their work place. Supervisor and HSE personnel shall provide guidance and
assistance for mustering at the correct muster point.
All members of the respective ERT teams shall report to their leaders. The leaders shall take
direction from the site OSC.

7.2. INITIAL INCIDENT INITIATION


In the event of any emergency the site HSE Supervisor or a member of the HSE department shall
assist the Site Manager to:
1) Evaluate the scale of the incident and decide whether additional resources are required to
adequately deal with it.
2) Immediately notify SCPC HSE Department of the scale and nature of the event.
3) Liaise with site supervision to withdraw any permits to work and ensure that all activities
are ceased as appropriate.
4) Give assistance to the site emergency response teams.
5) Liaise with site manager for the mobilization of any plant and equipment necessary for
dealing with the emergency.
6) Secure the area by setting up barriers or other means of preventing unauthorized access.
7) Ensure the OSC is appointed to manage the emergency response efforts.
8) When appropriate to do so, secure the site and commence the collection of evidence for
subsequent investigation
9) Prepare a full report in accordance with SCPC’s incident response procedures.

7.3. EVACUATION PROCEDURES


In the event of a fire, major plant failure, explosion, bomb threat or the need to evacuate the
plant, the actions listed below should be followed.
1) On the continuous sound of the alarm siren (bells), stop all activities, turn off all
equipment and vacate the building or area without delay, by the nearest exit
2) Move quickly, but do not run.
3) Do not return to a work area to collect belongings.
4) Do not overtake others along the route.
5) Assemble in the designated muster point.
6) At muster point, warden to reports to Site HSE Supervisor.

7) Do not enter the building or work area under any circumstances until the all clear signal is
given.
All new staff, as part of their induction, shall be given instructions on the evacuation procedures
for the site.
All visitors and Subcontractors are to be advised of the Site's Evacuation Procedure and the
location of Muster Point when they are admitted to the site.

7.4. MUSTER POINT


Designated muster points have been marked on the site layout maps at Appendix G. These map
and evacuation procedure will be displayed on notice boards throughout the site. Muster point
wardens should be appointed to manage and be responsible for operations at each muster
point.

7.5. INFORMATION AND DATA MANAGEMENT CONTROL

8.0. POST-EVENT MANAGEMENT


Following incidents which involved the implementation of the ERP and involvement of the
emergency response personnel, the HSE manager should ensure that a copy of all the incident
paperwork including the chronological log is kept secure (sealed/signed) for future
investigation and reports.

8.1. POST-EVENT ANALYSIS


A report analyzing the management of the incident should be prepared, providing the following
information:
1) An outline of the incident
2) Factors which lead to the decision to call out the ERT’s
3) Time taken to call in the ERT
4) Organization and management of the Emergency Operations Centre.
5) The management of information flow, both internally and externally to the company.
6) The handling of information
7) Effectiveness of training and competency of the ERT’s and contractor’s emergency
response framework to meet the response requirements.
8) Conclusions and recommendations for improvement.
