PARENT CONSENT FORM

The Department of Education issues the amendment to DepEd Order (DO) No. 13, s. 2018 titled Implementing
Guidelines on the Conduct of Remedial and Advancement Classes During Summer to assist schools in
identifying learners in need of instructional support and layout mechanism by which remedial classes during
summer can be maximized to address learning gaps brought primarily by the abrupt pedagogical shift due to
the pandemic. Hence the K to 12 Basic Education Program. DO 13, s. 2018 used the term summer classes for
School Year 2021-2022 falls from July to August, the term summer classes shall be amended to End-of-
School-Year (EOSY) classes. The end-of-school year classes for SY 2021-2022 are consistent with DO 029,
s.2021, School Calendar and Activities School Year 2021-2022 states that remedial, enrichment, and
advancement classes for public schools shall start on July 24, 2022, Monday, and end on, August 12, 2022,
Friday. (DO) No. 13, s. 2018 & DO 029, s.2021).

Summer Remedial Face to Face Classes

Hereby acknowledge that I have been informed of the details of the Conduct of Remedial Face to Face
Classes.

I understand that Lumayang Elementary School shall implement the minimum public health standards set by
the government to minimize the risk of the spread of COVID-19, but if cannot guarantee that my child will not
become infected with COVID-19 given that it is highly contagious.

I acknowledge that my child/children’s I persons attendance in this activity will include associating with
teachers, fellow learners, school personnel, and other people inside and outside that may put my child at risk
of COVID-19 transmission, notwithstanding the precautions undertaken by the school.

I acknowledge that my child/ children’s participation in his activity completely voluntarily. While there remains
the risk of possible transmission to my child/ children and o the members of my household, I freely assume the
said risk to attend the school under this activity, the Summer Remedial Face-to-Face Classes.

To the extend allowed by law and rules, I hereby agree to wave, release and discharge any and all claims,
causes of action, damages and rights against the school and its personnel as well as officials relative to the
conduct of the activity.
