Dear Parent/Guardian:

This is to inform you that Kawit Elementary School has been selected as one of the schools
countrywide to perform a PILOT TESTING FOR LIMITED FACE-TO-FACE (F2F) CLASSES in
accordance with the President of the Republic of the Philippines, Rodrigo Roa Duterte's permission.

This pilot will give valuable experience and learning that will be beneficial in making the final
recommendation on the return of face-to-face classes that DepEd wants to make.

In accordance with this, we'd like to let you know that your son/daughter is a part of the pilot
testing for face-to-face classes. Rest assured, the school will do everything possible to adhere to the
Inter-Agency Task Force's safety guidelines (IATF).

I have read and understood the information regarding the request and my consent.
