3. On the scheduled enrolment for your program, you should be ready to submit the original copies of the

a. School Form 10 / Form 138 for 1st Year Students and Transcript of Records (TOR) for
Transferees;

b. MCAT Rating (this notice);

c. Philippine Statistics Authority (PSA) issued Birth Certificate;

d. Certification of good moral character from the Principal or Guidance Counselor;
e. Certification from the University Physician indicating physical and mental fitness;
