HOME VISITATION ACCOMPLISHMENT REPORT

Home visitation programs provide structured services in order to alter the knowledge,
beliefs and/or behavior of children and caregivers or guardians in the caregiving environment,
and to provide parenting support. Home visitation is basically teachers visiting learners in their
homes and talk with the parents to identify the problems why their child is no longer attending
school. By home visitation, teachers will gather more of the pupils and their family background.

Poloyagan Integrated school found it needful and necessary that teachers who performed
home visits will improve learners’ classroom behavior. The reported behavior changes also
resulted in improved work habits, which then positively impacted the learners’ academic
achievement.

The school’s home visitation practices and programs purposely increase parenting
knowledge, parent child interaction, and school involvement. It also helps prevent child abuse
and neglect which will result to drop outs and failures at the end of the academic year. Most
importantly, it also helps increases school readiness & success. Additionally, this will also
promote early literacy which is a cognitive skill vital to learning in all subject areas.

As early as October of 2021, the teaching staff together with the parents convened for a
meeting and orientation regarding the attendance of the pupils and when to conduct home
visitation. The parents were also oriented on the school policy, learning expectations and
expected positive outcomes. To constantly reach out the parents, there will be a quarterly
meeting at the end of every quarter. Home visitation forms will be prepared by the teachers
which include the reasons for home visitation and the remarks and agreement between the
parents and the teachers.

All school year round, the teachers conducted home visitations to learners who are
frequently doing absences, learners with low performance in answering their modules and for
those who did not submitted and claimed their modules on time. Checked Self-learning modules,
Class Records and Individual Learning Monitoring Plan served also as basis of the teachers in
determining the learners to be visited. Pictures taken up served as documentation for the activity.
Barangay officials helped the teachers to be open guides to the actual residence of the family.
Parents affixed their signatures to the home visitation forms as a manifestation of their positive
and strong support to the schools’ activity to increase learner’s performance.
As an outcome, home visitation conducted have improved the learner’s performances.
With the collaboration of the barangay officials and the community, Home Visitation of the
school year 2021-2022 yielded a positive result and enlightened the parents, learners and
teachers.
