Definitions:
Unsafe Condition:
A situation that deviates from an accepted safe condition and could cause injury, interruption of work, damage, spillage or contamination.
Unsafe conditions are about equipment and physical situations.

Unsafe Act:
Action of a person which deviates from the required safe practice and creates a hazard to themselves or others. Unsafe acts are about
people, what they do or what they failed to do.

Note: Mention your actions taken against unsafe acts and unsafe conditions in “ACTION/STATUS” column in Unsafe Acts & Unsafe
Conditions Section. If an unsafe condition is registered in ME2, mention the ME2 reference number in “ACTION/STATUS” column. If
you have any suggestion to an unsafe condition/unsafe act, register the suggestion separately in suggestion scheme.

Typical checks during SOR:


Housekeeping: Is workplace neat? A place for Risk Assessment: Is job routine or non-routine? Is Personal Protective Measures: Minimum PPE

SC Normal operating conditions were observed at MF3. No abnormality was observed.

SA Arun is an honest Operator. He explained what he knew and informed what he was not
familiar with. Have good understanding of the working of MF3 CPP and DPP portions
during the SOR. Very informative. He knew the operations that was going on them.

SC TK0501 which was in operations loading to MT Nordic Skier at SPM was observed with no
abnormality. Tank surrounding was inspected, tank manholes were observed and found no
leak, all grounding cables were intact, sound from tank pipeline in operations was normal,
all valves connections and flanges were observed with no leak. Normal operations in
progress.

SC Fire extinguishers sampled was with valid tags.

SC Safety shower and eye wash sampled was in operating condition.

UA Housekeeping can be improved. Waste plastics found at Waste plastics were picked up at
various locations. various locations and disposed in
the non-oily bins / pick-up.

UA Two discarded mineral water bottles found at MF4. Mineral bottle found were
disposed accordingly.
