Definitions:
Unsafe Condition:
A situation that deviates from an accepted safe condition and could cause injury, interruption of work, damage, spillage or contamination.
Unsafe conditions are about equipment and physical situations.

Unsafe Act:
Action of a person which deviates from the required safe practice and creates a hazard to themselves or others. Unsafe acts are about
people, what they do or what they failed to do.

Note: Mention your actions taken against unsafe acts and unsafe conditions in “ACTION/STATUS” column in Unsafe Acts & Unsafe
Conditions Section. If an unsafe condition is registered in ME2, mention the ME2 reference number in “ACTION/STATUS” column. If
you have any suggestion to an unsafe condition/unsafe act, register the suggestion separately in suggestion scheme.

Typical checks during SOR:

Housekeeping: Is workplace neat? A place for
everything and everything in its place? Slip/trip hazard?,
etc.
Vopak Fundamentals on Safety: Is activity
being performed as per the Vopak Fundamentals on
Safety? Transfer of Product, MOC, PTWS, LOTO,
Confined Space Entry, Work at Height, Excavation,
Motorized Vehicle, etc.
Permit to Work System: Is there permit for the
job (for all non-routine jobs)? All sections in the permit
filled in and duly signed by all? Any reference to risk
assessment in the permit? Are all control measures being
followed? Is Permit Acceptor supervising the job at
site?, etc.
Jetty/SPM Safety: Life ring with rope available in
the vicinity? Life jacket required, if so, is it followed?
Corroded/damaged jetty/SPM structure (piles, mooring
hook, fender, gangway, mooring ropes, etc.)? High
wind/high swell? Vessel rolling dangerously? Any open
edge to sea? No or improper positioning of fire wire?
Excessive vessel trim/list?
Risk Assessment: Is job routine or non-routine? Is
there risk assessment? Is a copy of risk assessment
available at site (for non-routine jobs)? Are control
measures being followed?, etc.
Behavioral Safety: Am I working safely and
following all safety measures? Is my colleague working
safely? Did I appreciate my colleague for working
safely? Does my colleague need to be alerted about his
unsafe acts and if so, did I alert him?, etc.
Safe tools and equipments:
A standard tool? Suitable for work? Used properly? In
safe condition (mushroomed head/corroded/worn out,
etc)? Are pump/valve seals leaking? Is equipment at
inaccessible area? Any abnormal sound in the
equipment?, etc.
Confined Space Entry: Is permit available at
site? All sections in the permit filled in and duly signed
by all? Any reference to risk assessment in the permit?
Are all control measures being followed? Is Permit
Acceptor supervising the job at site? Is manhole watcher
available near manhole with radio? Is rescue equipment
available at site? Is system isolated from the confined
space? Is ventilation/illumination in confined space
adequate?, etc.
Personal Protective Measures: Minimum PPE
(safety helmet, safety glass, safety shoes, full-sleeve
coverall) Additional PPE (if required): splash-proof
glass/visor, chemical resistant coverall/gloves, life
jacket, full-body harness, hand gloves, etc.
Procedures: Is Standard Operating Procedure
available for this job? Is the procedure suitable for the
work? Is procedure as per Vopak Standard? Is the
procedure known and understood? Is the procedure
being followed? etc.
Explosion-proof instruments/equipments:
Is instrument/lighting/equipment selected as per the Zone
classification (Zone 0/1/2)? Is the equipment damaged /
defective? etc.
Work Environment: High humidity and
temperature, corroded platform, weak roofing, high
wind, poor illumination, slippery surface, dust,
hazardous vapors, noise, poor ventilation, hard- to-
approach area, no handrail/mid rail/toe guard, Type &
number of Movements, Load handled, Design of the
workplace, Vibrations, Unsafe ergonomics, improperly
adjusted workstations/chairs/tables, frequent lifting,
repetitive or awkward movement, etc.

SC Normal operating conditions were observed at MF3. No abnormality was observed.

SA Arun is an honest Operator. He explained what he knew and informed what he was not
familiar with. Have good understanding of the working of MF3 CPP and DPP portions
during the SOR. Very informative. He knew the operations that was going on them.

SC TK0501 which was in operations loading to MT Nordic Skier at SPM was observed with no
abnormality. Tank surrounding was inspected, tank manholes were observed and found no
leak, all grounding cables were intact, sound from tank pipeline in operations was normal,
all valves connections and flanges were observed with no leak. Normal operations in
progress.

SC Fire extinguishers sampled was with valid tags.

SC Safety shower and eye wash sampled was in operating condition.

UA Housekeeping can be improved. Waste plastics found at Waste plastics were picked up at
various locations. various locations and disposed in
the non-oily bins / pick-up.

UA Two discarded mineral water bottles found at MF4. Mineral bottle found were
disposed accordingly.
