BAC Resolution Approving the Bid Evaluation Report and Recommendation for Award of
Contract of Service for the Fabrication/Installation and Maintenance of Trashtraps along Pampanga
River particularly at Brgy. Sto. Tomas, Sasmuan, Pampanga under the Manila Bay Rehabilitation
Project

RESOLUTION NO. MB 2021-10

WHEREAS, the Provincial Environment and Natural Resources, Office, Brgy. San Antonio,
Guagua, Pampanga advertised the Request for Quotation (RFQ) for the Fabrication/Installation and
Maintenance of Trashtraps along Pampanga River particularly at Brgy. Sto. Tomas, Sasmuan, Pampanga
under the Manila Bay Rehabilitation Project on January 20, 2021 at PhilGEPS Website;

WHEREAS, the mode of procurement to be followed is Negotiated Procurement – Small Value

Procurement (Sec. 53.9);

WHEREAS, the Approved Budget Cost (ABC) is One Hundred Fifty Six Thousand Four
Hundred Thirty Five and 20/100 Pesos (Php 156,435.20);

WHEREAS, upon examination of the submitted quotation by the bidder, BIG 3 ENTERPRISES
owned by MS. MARY JANE ALVENDIA QUIWA, with postal address at 2765 Rose Avenue, Purok 10,
Santa Lucia, City of San Fernando, Pampanga, was found to have the Lowest Calculated Bid for all the
items, to wit;

WHEREAS, upon further evaluation conducted, the Technical Working Group (TWG) for
Forestry, Protected Area, Manila Bay, Infrastructure and Land Management Related Projects
recommended through Memorandum for the PBAC Chairman dated February 16, 2021, that the contract
be awarded to BIG 3 ENTERPRISES through Negotiated Procurement – Small Value Procurement.

NOW THEREFORE, We, the members of the Bids and Awards Committee, hereby RESOLVE
as it is hereby RESOLVED:

a.) To declare BIG 3 ENTERPRISES, the Bidder with the Lowest Calculated and Responsive
Bid for Contract of Service for the Fabrication/Installation and Maintenance of Trashtraps
under the Manila Bay Rehabilitation Project

b.) To recommend for approval by the Provincial Environment and Natural Resources Officer
the foregoing findings.

With reference to the Contract of Service for the Fabrication/Installation and Maintenance
of Trashtraps along Pampanga River particularly at Brgy. Sto. Tomas, Sasmuan,
Pampanga under the Manila Bay Rehabilitation Project in the amount of One Hundred
Fifty Six Thousand, Four Hundred Thirty Five and 20/100 Pesos (₱ 156,435.20), entered into
by and between you and the Department of Environment and Natural Resources PENRO, San
Antonio, Guagua, Pampanga, you are hereby advised to proceed with the works agreed upon in
the Contract.

You are further advised that all activities, prescription standards, schedules shall be in
accordance with the attached schedule activities agreed in the Contract of Service. Any deviation
from those should necessity arise, should be first cleared from the undersigned or his authorized
representative.

Dear Ms. Quiwa,

We are happy to notify you that the Contract of Service for the Fabrication/Installation and
Maintenance of Trashtraps along Pampanga River particularly at Brgy. Sto. Tomas, Sasmuan,
Pampanga under the Manila Bay Rehabilitation Project is hereby awarded to you as the Bidder
with the Lowest Calculated Responsive Bid at a Contract Price of One Hundred Fifty Six
Thousand, Four Hundred Thirty Five and 20/100 Pesos (₱ 156,435.20).

You are therefore required, within ten (10) days from the receipt of this Notice of Award, to
formally enter into contract with us, and to submit the Performance Security in the form and the
amount stipulated in the Instruction to Bidders. Failure to enter into the said contract or provide
the Performance Security shall constitute a sufficient ground for cancellation of this award and
forfeiture of you Bid Security.
