MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This memorandum of agreement, is entered into by and between the


ROSARIO NATIONAL HIGH SCHOOL,Schools Division of Santiago City, an agency
of the National Government of the Republic of the Philippines with address at
Camacam St, Victory Norte , Santiago City represented by Emerita T. Mawirat,
Secondary School head, herein referred to as the “FIRST PARTY;”

The learner, represented by


___________________________________, by his/her parent/guardian with address
at ________________________, herein referred to as the “SECOND PARTY.”

WITNESSETH:

WHEREAS, in response to the ongoing Community Quarantine


Classifications and No Face-to-Face policy of the National Government, the
Department of Education implements the Basic Education Learning Continuity Plan
(BE-LCP) that aims to provide basic education services by implementing various
learning modalities such as modular distance learning;

WHEREAS, Pursuant to DepEd Memorandum DM-CI-2O2O-OO 162 entitled


Suggested Strategies In Implementing Distance Learning Delivery Modalities
(DLDM) for School Year 2O2O-2O21, the Schools Division Office of Santiago,
through the generous provision of the Local Government Unit headed by Mayor
Joseph S. Tan, provide devices for the implementation of digitized modular distance
learning modality and eventually online distance learning modality.

NOW, THEREFORE, for and in consideration of the foregoing agreement, the


parties hereto do hereby agree as follows:

SECTION 1. SCOPE AND COVERAGE. Grade 7 public school learners of Rosario


National High School, Schools Division of Santiago City who are officially enrolled
and verified from the Learner Information System (LIS), identified by their respective
class adviser as not financially capable of acquiring such device and which are
included in the list submitted to the Division Office shall receive one (1) tablet with a
and with the following specifications:

Operating System: Android 10.0 with GMS Battery: 2,500 MAH


CPU: A50 3G Bluetooth 3.0
RAM: 2 GB WIFI: 802.11 B/G/N
Storage: 16 GB Display: 8” 1024 x 600 HD
Camera (Front and Rear)

SECTION 2. OWNERSHIP. The device shall remain the property of the school.
Parents or learners have no ownership, interest, or right to title in the tablet device;

SECTION 3. RESPONSIBILITY OF THE FIRST PARTY. It shall be the over-all


responsibility of the Rosario National High School to strictly monitor and evaluate the
implementation of this program. The following units under the schools have the
following responsibilities:

a. The School head and Grade 7 advisers shall monitor and evaluate the
implementation of the distribution of the tablets;

b. The School Supply and Property Officer shall monitor and evaluate the
property accountabilities of all personnel involved in this endeavor;

c. The School Information Technology Officer and his team shall monitor and
evaluate the overall technical aspect from distribution and downloading of
learning materials;

d. The School head under the supervision of Curriculum Implementation Division


(CID) through the Education Program Supervisors headed by the CID Chief

shall be responsible for the quality assurance of the learning materials and
shall monitor the installation of the same onto the device.

e. The School Heads shall monitor and evaluate the school-based level
implementation of distribution and downloading of learning materials.

f. The school shall be responsible for the uploading of all learning materials in
the tablet, good for a period of time the school deemed appropriate;

g. The school shall retrieve the device every month for monthly installation and
uploading of learning materials.

SECTION 4. ACCEPTABLE USE OF TABLET AND ACCOUNTABILITY OF THE


SECOND PARTY. The following are the acceptable use of the tablet and the
accountability of the learner’s parent/s:

a. The tablet shall not be used for personal profit or non-profit purposes such as
advertising, rentals, selling, or other such uses;

b. The tablet shall not be used in unlawful activities;

c. The tablet shall be handled with care, protecting its functionality and
appearance;

d. If the tablet is damaged as a result of accident and force majeure while it is


being used in normal operating conditions, there shall be no liability on the
part of the parent/s but they shall notify the school immediately;

e. The parent/s shall be responsible for the replacement or for the full
replacement value of the tablet when found damaged as a result of use
outside the parameters established herein or if it is lost or stolen due to
negligence;

f. The Parent/s shall be responsible to return the tablet to school in as good


condition as when received, except for reasonable wear and tear. During the
academic period and prior to return, the parents agree to assume all
responsibility for maintenance and repair.

g. There shall not be installation of other software onto tablet unless specifically
authorized by the school;

h. There shall not be intentional transmission of viruses and other such


malicious programs onto tablet;

i. There shall not be modification, upgrade, or attempt to repair tablet without


the expressed permission of the school.

SECTION 5. RETURN OF THE TABLET. At the end of School Year/Calendar Year,


a date will be announced on which the parent/s shall be instructed to return the said
device to the school being the rightful owner.
