First Aider Appointment

APPOINTMENT AS FIRST AIDER

NAME OF ORGANISATION: Young And Independent

In term of General Safety Regulation 3 as incorporated under the Occupational Health and Safety Act,
1993 I hereby appoint you Naeem Hamid

as a first aider for the Evander And Embalenhle Area/Department.

As the appointed first aider it is expected of you to:

1. Familiarize yourself with the exact location of all the first aid equipment under your
control;
2. Ensure that your name appears at the first aid box under your control;
3. Ensure that you are always in possession of a valid certificate of training in first aid;
4. Treat all injured staff in the area for which you have been appointed in accordance the
training received;
5. Conduct monthly inspections of the first aid box / boxes under your control as to ensure
that these boxes are stocked with all items as prescribed by the legislation;
6. Discard of all Hazardous Biological matter in the appropriate manner; and
7. Complete the Treatment Record when applying first aid to a person
