TRAINING CONTRACT

KNOW ALL MEN BY THESE PRESENTS:

This TRAINING CONTRACT is made, executed and entered into by:

 
       Prime Innovations School of South Davao INC. hereby represented with its Coordinator
employee, a duly organized entity existing under laws of the Republic of the Philippines with
office and postal address at Parallel St, Brgy. Tiguman, Digos City, Davao del Sur, hereinafter
referred to as the ”COORDINATOR”
-and-
 
_________________________________ of legal age, single/married (underline civil status),
Filipino and a resident of _________________________________, hereinafter referred to as the
“TRAINEE”.

WITNESSETH;

WHEREAS, the TRAINEE is an applicant for work position with the Company of

coodinator;

WHEREAS, the TRAINEE needs to upgrade his teaching skills and capacity to reach the
standard of the Company;

WHEREAS, the Coordinator has the obligation to conduct training program to the
TRAINEE and the latter will sumbit himself to undergo training and coaching by the former;

WHEREAS, the training program must be completed both by the TRAINEE and the
Coordinator in view of the employment standard required by the Company;

NOW THEREFORE, for and in consideration of the stipulations herein stressed out in this
agreement, the following terms and conditions are hereby voluntarily entered into between
parties:

THE CONTRACT

1. Opportunity for employment is made available and assured by the above-named

Company, Prime Innovations School of South Davao INC , after exemplary
compliance by the TRAINEE of the regulations and conditions herein specified.

2. Training shall be within 7 days or beyond depending upon the recommendation of the
Coordinator.

3. There will be No Allowance provided by the company to the TRAINEE for the entire
training period.

4. TRAINEE, upon signing of this contract, waive its right to claim any form of
compensation or damages should undesirable incidents may occur during the training
period.

5. Should the TRAINEE withdraws the training program must notify the Coordinator
properly in writing and site his compelling reasons thereof.
 6. The Company or the Coordinator may terminate the Training Program at anytime at the
discretion of the former if any of the provisions herein is/are violated.

7. This Contract shall take effect immediately on the date the parties affix their signatures
below.
