AFFIDAVIT OF ARREST

We, P/Msgt. Jimson S. Garcia and P/Msgt. Joseph V. Licuan of legal age,
bonifide member of the Philippine National Police and presently assigned at Pasuquin
Municipal Police Station, after having been duly sworn to in accordance with the law, do
hereby depose and state:
1. That on August 20, 2022, at around 8:30 in the evening our station
received an information from the reporter John Lexter Calilan y Angeles of Sitio Bocal,
Brgy. 10 Estancia, Pasuquin, Ilocos Norte that was a clubbing incident transpired at the
said place.

2. That, acting on that information, the officer in charge directed us to

respond at the reported place of incident to verify the veracity of the said information
and if affirmative, to conduct an ocular investigation;

3. That, upon arrival at the place of incident, at about 9:00 in the evening, we
were personally informed by one of the witnesses. Likewise, we were able to arrest the
suspect after the incident with the weapon used;

4. That, we immediately informed our station confirming this incident; and

thereafter, Operating team was created and manhunt operation was lunched.
Information gathering, as to the location of the suspect, was enhanced by tapping the
intelligence community. Informants were tasked to locate the whereabouts and activities
of the suspect. An asset of this station reported that the suspect was arrested in his
residence at Sitio Bocal, Brgy. 10 Estancia, Pasuquin, Ilocos Norte;

5. That, we immediately identified ourselves to Fernando Failano y Valdez

and arrested the same; and we likewise informed them the reason of their arrest and of
their constitutional rights;

6. That, the suspect submitted himself to us peacefully; and further identified

himself as one Fernando Failano y Valdez, 25 years old, single and a resident of Brgy.
10 Estancia, Pasuquin, Ilocos Norte; and that, we immediately brought him to our
station for investigation and proper disposition; and

7. That we executed this affidavit to attest the veracity of the aforementioned

facts and to support the filing of appropriate charge/s in court against the arrested
suspects;
