BEFORE CATHETER INSERTION

Patient Preparation

1. Verify the order.


2. Prepare the materials needed.

3. Identify the patient, explain the procedure,


and ask for iodine allergies.

4. Ensure privacy and good lighting.

6. Perform hand hygiene, don clean gloves, and


cleanse the perineal area with soap and
water. Rinse and dry.

7. Remove gloves and perform hand hygiene.

DURING INSERTION

Draping the Patient

8. Open the sterile catheterization kit on a clean


bedside table.

9. Put on sterile gloves and drape the patient.

Pick the underpad, ask the patient to raise


hips, and slide the drape under patient’s
buttocks.

Pick fenestrated drape and place over


perineum with the hole over the labia.

Materials Preparation

10. Prepare the antiseptic solution (using iodine


swab or cotton balls with betadine using a
sterile forcep).

11. Attach the saline-filled syringe to the side


port of the catheter and inflate the balloon.
After, deflate the balloon and leave the
syringe connected to the port.

12. Apply sterile lubricant to the first 2.5 to 5


cm ( 1 to 2 inches) of the catheter tip.
Attach the catheter to drainage system, and
ensure the drainage bag emptying port is
clamped.

13. Bing the kit to the sterile field between the


patient’s legs.
Urethral Meatus Preparation

14. With non-dominant hand, identify meatus


by retracting the labia. Hold this position
throughout the procedure.

15. With sterile dominant hand, clean the


meatus with the antiseptic solution moving
from top to bottom. Allow the antiseptic to
dry.

Catheter Insertion

16. Ask the patient to bear down, with sterile


dominant hand, slowly insert the catheter
into the urethra until urine flows. Then
advance the catheter 1-2 inches more. (Do
not force the catheter through the urethra).

While inserting: Ask the patient to take slow


deep breaths until the initial discomfort has
passed.

17. Hold the catheter with the non-dominant


hand; use the sterile dominant hand to fully
inflate the catheter balloon. Remove
syringe.

18. Gently pull the catheter until resistance is


felt.

19. Secure catheter on patient’s inner thigh


with securement device (strap or plaster).

20. Ensure the tubing is not kinked and secure


the drainage bag to the bed frame below
bladder level (but not resting on the floor).

AFTER PROCEDURE

21. Dispose used supplies. Place syringe in


sharps container.

22. Remove gloves.

23. Assist patient to a comfortable position.

24. Perform hand hygiene.

Documentation
25. Document—type and size of catheter and
balloon, amount of fluid inserted in the
balloon, patient’s reaction to the procedure,
amount and characteristics of urine, name of
person performing the insertion, and the
time and date it was completed.
