(FAC?) 19th June: Foot injury. 4” spool was being transferred from cellar deck to
main deck using platform crane operated by WOP. IP was part of the platform
rigging crew on the main deck. When the spool made contact with the deck, it
bounced and IP was close by (LOF) and spool made contact with IP foot.
