33. Import Conditions

1. I declare that these products will be used in accordance with all Philippines quarantine requirements as may be
specified in the VQC to be issued for the importation and other DA rules and regulations relevant thereto.
2. I subscribe and declare to follow all the applicable provisions of the DA rules, regulations, standards, on the importation
of meat and meat products into the Country and other DA rules and regulations relevant thereto.
3. I declare that at the time of the filing of this application, the meat and/or meat products that are to be imported are still in
the country of origin and have not yet boarded in a vessel/aircraft.
4. I declare that the information I have provided herein are true and accurate to the best of my knowledge and belief.

This importation is subject to DA existing rules and regulations and to the following conditions stated at the back hereof.
This Clearance shall be valid for only one shipment. Failure to import the quantity allowed will result in a forfeiture of the
right to import the balance.

34. Other Condition/Requirement

IMPORT TERMS AND CONDITIONS FOR DOGS AND CATS INTO THE PHILIPPINES Applications to import pet animals shall be
subject to the following conditions: 1. That the animals should be accompanied by a health certificate written in English or have a
certified English translation, issued shortly before shipment by the proper veterinary authority of the country of origin. 2. The health
certificate should contain the following information: a. description of the animal b. vaccination details (date of vaccination, name of
vaccine, manufacturer, batch number, expiry date) c. clinical examination that the animal/s have been carefully examined and found
to be free from and have not been exposed to dangerous communicable animal diseases; 3. That the animals should be
accompanied by a valid vaccination certificate against the following diseases: a. For dogs: rabies, canine distemper, infectious
hepatitis, leptospirosis, canine parvovirus, canine parainfluenza for dogs; b. For cats: rabies and feline panleukopenia. 4.
Vaccinations should have been administered at a minimum of 14 days before travel. 5. Anti-rabies vaccination of pets from rabies-
free countries based on current standings with the World Organization for Animal Health (OIE) could be waived. 6. The vaccination
certificates must include all of the following information: a. Name and address of owner b. Breed, sex, date of birth (approximate
age if date of birth unknown) c. color, markings, and other identifying information for the dog d. Date of vaccination and vaccine
product information e. Expiration date/validity date of vaccination f. Name, license/registration number, and signature of veterinarian
who administered the vaccination 7. That the animal/s has/have an d medication against internal and external parasites; 8. If the
animal arrived without valid SPS Import Clearance, valid health certificate issued by the veterinary authorities or vaccination
certificates, they shall be subjected with the penalties written in BAI Memorandum Circular 43 series of 2020. 9. Only dogs and cats
that are four months old and above at the time of SPSIC application are allowed to be imported into the Philippines; 10. Dogs/cats
should be vaccinated against rabies at minimum age of 12 weeks of age; OR Dogs/cats vaccinated against rabies earlier than 12
weeks of age shall be given a booster shot with owners consent before applying for SPS Import Clearance. 11. The approved SPS
Import Clearance is valid only for 60 days from date of approval. Should the animal/s has/have not shipped out before the must ship
out date as written in the SPS Import Clearance, the importer must secure another SPS Import Clearance. Extension of validity of
SPS Import Clearance is prohibited. 12. The importer should pay the inspection and SPS Import Clearance fees at the BAI
Quarantine office at the Port of Entry upon arrival. SPS Import Clearance Fee P100.00 Inspection Fee - First Mo heads P250.00 - In
