This Student Assessment Booklet includes all your tasks for assessment of CHCCCS023 Support
independence and wellbeing.

ABOUT YOUR ASSESSMENTS


This unit requires that you complete 4 assessment tasks. You are required to complete all tasks to
demonstrate competency in this unit.

Assessment Task About this task

Assessment Task 1: Written questions You must correctly answer all questions to show that you
understand the knowledge required of this unit.

Assessment Task 2: Case studies There are three case studies in this task. You are to read and
complete the questions related to each.

Assessment Task 3: Role play You are to participate in a role play with your assessor.

Assessment Task 4: Workplace There are two parts to this observation task:
observation  Part A: Students are to be observed working with three
clients (include their family/carers) to support their
independence and wellbeing.
 Part B: Students must demonstrate their ability to report
two fictional client situations by following their
organisation’s relevant policies and procedures.

Workplace hours The student must have performed the activities outlined in
the performance criteria of this unit during a period of at least
120 hours of direct support work in at least one aged care,
home and community, disability or community service
organisation.

Wellbeing is the state of being comfortable, healthy and happy. In this growing economic world, finance
affects every sector of our life. To be healthy and get proper medical care our financial situation should be
good. If we are financially strong, we can choose from different options which will suit us the most. For
example, if our financial situation is good we can purchase organic and fresh foods from the market
which makes our health better but if our financial situation is not so good then we have to purchase low
quality food which may harm our health. When a financial situation is good then we can fulfil our required
body nutrients and make our health better. The financial situation is directly proportional to our well-being.
When we feel sick or body malfunction then we can go to a better hospital for a check-up if the financial
situation is good. And if the financial situation is bad then we ignore small health problems which may
lead to a big problem in future. So financial situation directly affects well-being.

b) How can a person’s career or occupation affect their wellbeing? Provide an example.

We, humans, are social being and to live and sustain in any society we need to have a career and
occupation which provide us with our identity and give respect to us. There are different career options
from which we can choose and build our life on that. When we enjoy our job then we can give our 100%
to fulfil the objective of a job but if we don’t enjoy it then it can bring psychological illness. If our job pays
well then we can be satisfied and be happy but if the pay is less then we struggle to sustain our life which
will directly affect our mental wellbeing and physical wellbeing too. If our occupation is stress-free then
our health will be good mentally. In today’s world occupations are divided into physical and mental work.
So if we do physical work then it can make our physic healthy but mental health could be bad. But if do a
mental job then our physical health is directly affected. So we should keep- a balance between physical
and mental health while working.

c) In what ways could the financial situation of a person with a disability impact their physical and
psychological health and wellbeing?

A person with a disability is already struggling with their physical daily work. Due to the development of
technology and infrastructure many tools have been developed to help such disabilities. Every person
needs good finance to survive so a person with a disability also needs to work to make their financial
situation better. The financial situation is directly related to the physical and psychological well-being of a
person. A person with a disability needs more pay than other normal people because they need to spend
on their disability to work like normal people. Handicapped people can work better than a normal person
if their financial situation is better. A person with a disability is already struggling in their life so they should
not worry about other things too. If the financial situation of a disabled person is worse then they cannot
maintain their health properly which will affect their physical and psychological wellbeing. When the
financial situation is bad then mental pressure will increase which could hamper psychological wellbeing
and the person could be demotivated in their life which could bring mental illness. They could not be
motivated to live their life which could result in suicide. A person with a disability need someone to take
care of them if their financial situation is bad then they could not pay them and that person will leave them
which may create more problems

d) How may a sudden change in lifestyle affect a person’s wellbeing? Provide an example.

Sudden changes in life bring a mental and physical challenge in life, if we are able to handle the situation
then persons wellbeing will not be affected. If the person could not handle the situation then the person's
wellbeing is affected which can change their life. Life is a changing procedure, change can happen at any
point in time, if our mind handles it with care then our wellbeing is not affected. For many people, sudden
change brings fear of the unknown which causes a great deal of anxiety and stress. Sudden change
brings mental disorders which may cause personality change. For example, when we are at university we
are engaged in different physical activity and our life is much enjoyable but if suddenly we get a job and
engage ourselves in work then our physical activity decrease and mental pressure increases which bring
different in our personality. We look fat due to a lack of physical activity. Sudden changes in life can bring
big change and affect the wellbeing of a person. If we see some accident while walking then it will
hamper our mental health and we fear walking that path again. Such change can impact our daily
lifestyle and affect our well-being. We could be mentally disturbed.

QUESTION 3
What is self-actualisation? Provide an example.

Self-actualization can be defined as the complete realization of ones potential and the full development of
one's abilities and appreciation for life. It is the concept regarding the process by which an individual
reaches his or her full potential. Self-actualization is the highest level of psychological development where a
person knows his/her potential. According to the American dictionary; “Self-actualization is considered as
everyone's need or drives for the fulfilment of their own potential and talent”. For example famous
personalities like Mahatma Gandhi, Nelson Mandela etc are the live example who had anticipated self-
actualization by living purposefully for the betterment of society. They both have fought for the freedom of
their nation without any self- interest. Other examples can be the growth and development of employees in
their organization. Employees having self-actualization needs are more concerned about their growth and

development. They look for a more challenging task in their work and become more independent than
others.

b) If you suspected that a client was being abused, do you have an obligation to report it? Explain your
answer.

If there is any suspicion about abuse then at first we should conform about that abuse. I have no objection
to reporting such abuse but first I should conform about that abuse and the victim should also help me to
confirm it. If I report abuse and the victim denied it then I have to face a problem so I should first collect
evidence that supports my report and I should share that information with the victim also. I should consult
the victim and decide according to it. If the victim does not know about abuse then at first I should educate
him/her then they can report about that abuse by themselves. Awareness about abuse and types of abuse
in every organisation should be made which helps every client and employee. Abuse can bring different
psychological and social problems in individuals life so we should be very careful while handling such
cases. Any misunderstanding can bring a huge impact on any person's life. This matter could be sensitive
so we should handle it with care

QUESTION 6
a) List three health risks involved in poor dental hygiene.

1. Heart Disease/ stock risk: People with poor dental hygiene are two times more likely to develop heart
disease and arterial narrowing as a result of bacteria and plaque entering the blood stream through the
gums.

2. Respiratory Problems: Bacteria from poor dental health can travel through the bloodstreams to the lungs
where it can aggravate respiratory system, especially in patients who already have respiratory problems.

3. Diabetes: Many people with diabetes also have poor dental hygiene. This could lead to tooth loss
because people with diabetes are more susceptible to contracting infections.

b) List five ways that exercise improves health.

1. Help to control your weight: Exercise plays an important role in maintaining the weight and preventing
obesity. To maintain weight the calories we take must be equal the energy we burn.

2. Reduce the risk of Heart diseases: Exercise strengthens the heart muscle and improve the blood
circulation. The increased blood flow will raise the oxygen level in our body which reduce the risk of heart
diseases.

3. . Improve Mental health: While doing exercise body releases chemicals that can improve our mood and
make us feel relaxed which can help to reduce stress and risk of depression

4. Improve sleep: Exercise help us to fall asleep faster and stay asleep longer

5. Maintain sugar and insulin level: Exercise lower our sugar level and help insulin work better. This cut
down the risk of metabolic syndrome.

c) List three reasons why support with personal hygiene is important.

1. Good Hygiene saves lives: Personal hygiene are responsible for a huge amount of preventable deaths. It
id believed that adequate hygiene could saves millions of lives around world. Good hygiene can reduce the
risk of contamination of different diseases.

2. Personal hygiene makes us more attractive: When hygiene is good then we are better looking and
healthy which attract other people towards us. We also feel good when we meet person with good health
and hygiene.

3. Personal hygiene can increase the productivity: If our hygiene is good then we will be less sick which
reduce our sick leave at work which will increase our productivity in the work. When our health is good we
work with full concentration which will increase the productivity.

d) List four common diet-related health problems experienced by older people.

1. High blood pressure: Blood pressure is directly related to our diet. If we eat healthy then blood pressure
can be in balance but if the diet is not balanced then blood pressure will increase which may lead to
chronic heart diseases.

2. Diabetes: it is another diet related health problem if the diet is not balanced than diabetes is common in
today’s life style. The diet should be balanced and support our nutrient needs for the body.

3. Tooth decay: This is common duet related health problem in older people. Due to fast food and other
product our tooth are facing a lot of problems and we also don’t have good dental hygiene which lead to
tooth decay.

4. Overweight: Due to consumption of fast food and other product more people are being over weight and
facing lot of problem in their older life. Overweight makes people lazy and unattractive. It leads to many
health related problems.

e) List four things that can improve mental health outcomes.

1. Talk about the feelings: when we feel sad or feel unhappy, if we talk or share our problems with other
than we can make good mental health. Talking about the feeling can help us to stay in good mental health
and deal with times when we feel troubled.

2. Keep active: Doing exercise regularly can boost our self-esteem and help to sleep better and concentrate
well. Exercise keeps our brain and other vital organs healthy which helps to maintain or improve mental
health.

3. Eat well: Brain needs different types of nutrients to function well so good food can help to improve mental
health. The diet that is good for physical health is also good for mental health.

4. Drink sensibly: Alcohol are the drink which stimulate our brain’s function we should drink sensibly so that
our mind function well. If possible drinking should be stopped, if not possible than we should drink sensibly.

a) In one paragraph, describe a common myth that surrounds sexuality in older people, and why this myth is
not true.

There are many myths that surround sexuality in older people which are not so true. There is a myth that
only the young are sexually attractive which is not so true because older can be quite sexy. The features
that are attractive in young can be attractive at old age too. So the myth is not so true. Another myth is that
sexuality in later life is undignified. The fact is that it is healthy for older people to express their sexuality.
There are many social restrictions for older people regarding sex so they can’t express themselves sexually
in their late life, but in the changing society, we can see many examples that older people are expressing
themselves and getting married which has been a success in their life. Another myth is that men and
women lose their ability to perform sexually after a certain age. We can still have a satisfying sex life at any
age. There are many examples in the world where old aged people have been satisfying their partners. Sex
is the basic need of life like food and water so at any age we can perform good sex and satisfy each other
at any age. Age is just a number and we can achieve anything in life at any age we should have that
courage and positive mindset

b) In one paragraph, describe a common myth that surrounds sexuality in people with disabilities, and why
this myth is not true.

There are many myths that surround sexuality in people with disabilities. People with disabilities also need
sex in their life because sex is the basic need of life like food and water. Disabilities should not be the
reason to discriminate against any person because people with disabilities have achieved much success
that normal people feel hard to do. There is a myth that people with disabilities are not attractive. Sex is not
only for attraction, it is the basic need for life so people with disabilities are also beautiful and can attract
others sexually. Many people with disabilities are satisfied with their sex life so this myth is untrue. Another
myth is that people with disabilities don’t need sex education. Sex education is a very important topic in
today's world and every people need it. People with disabilities are also sexual beings and need sex
education. Sex education can empower people with disabilities with knowledge and information to have
safe and pleasurable sex. Another myth is that people living with a disability can’t have real sex. There are
many different ways that people can have sex that is not shown to us. Sex is for everyone and everyone
should enjoy it. If we cant have one short of sexual activity because of disability then another form of sexual
activity can suit us.

c) Describe at least three issues a client who is intersex or transgender may experience during their
everyday living.

1. Depression and anxiety : Transgender are also the people who are unique sexually so we should not
discriminate them because of such uniqueness. When we don’t treat them well or discriminate them then
can’t share their problems which lead them to depression and anxiety.

2. Self-harming: Transgender are treated differently in the society than others. They are treated in negative
way that demotivate them and they try to self harm themselves. We should accepted their uniqueness and
treat them like normal.

3. Substance abuse: Many people think that transgender do drugs and are involved in such activities. But
iot is not true, transgender are normal and we should accept them with their uniqueness. They are
mistreated sexually and substance abuse is faced by them.

Claude is a new client at New Greens Care Facility. He has come into care as he has had a stroke, and his
male partner Ray is no longer able to look after him. Claude is finding it very difficult to adjust to life at the
facility. Ray’s sister brings him in to visit Claude once a month as he is unable to drive, but she has never
been able to accept their homosexual relationship. After 20 minutes or so she insists on leaving.
Claude confides to his carer that he misses Ray terribly. He might as well not come to visit anymore as
Ray’s sister glares at them if they so much as hold hands and says they are disgusting. He yearns just to
share some love and affection.

1. What effect do you think Claude’s separation from Ray is having on his emotional wellbeing?

Emotional wellbeing is the ability to produce positive thoughts, emotions, moods and feelings. Emotion is a
very important factor in anyone’s life. We, humans, are bound by emotion and love the positive emotion.

Claude she must get negative felling due to which she tried to get his brother back as soon as possible.

Apart from supplying a trundle bed care centre can make a good environment where they feel safe and can
have a private and comfortable talk. Others should not be allowed to their room. Their room should be safe
and others should not have access to that room. They should not be disturbed and proper medical supplies
should be already provided. If possible their room should be far from others so that they don’t get disturbed.
There should be a proper supply of contractive which they may need at any point of time. The privacy of
their relationship should not be shared with anyone other which may bring some crisis. Proper security

You stop in to check on Claude and Ray and notice that they have set out and lit a lot of candles. While you
think that is very romantic, you ask them to extinguish a number of candles, specifically those on the
window sill and some on the floor.
Ray starts to put them out but Claude asks you why they need to.

7. Explain what you would say to Claude, taking into consideration your organisation’s relevant workplace

As being the caretaker I should be very careful in safety measure. At first, I should be having the safety of
each member of the centre. I should explain to Claude that it can cause serious problems like fire which will
affect everyone other who are in the centre. the organisation has provided all the facilities that they needed
and it is the client's responsibility to maintain those facilities. If an organisation grant some privilege to
anyone then in the coming day's others could ask for other privileges which the organisation cant afford. An
accident can happen at any place at any time so it is my responsibility to make everyone safe. Due to
candles there could be fire and risk others life. At first, I should try to convince Claude that his action can risk
others life.

8. Claude asks you what else they could use instead of all the candles. Identify two alternatives that would

Claude is much happier now Ray is able to visit regularly. However the other day they got a bit carried away
in the dining room and offended some of the other residents who complained that their behaviour was

10. How would you deal with this issue?

At first, it's my responsibility to take care of all the clients. I should maintain the peace and security of that

co-workers, how do you go about making sure you do not impose your views onto others?

Every person has their own view on sexuality. I also have mine. People are different from each other in
many ways. We are from different backgrounds which makes us different and unique. We love to see what
we believe but some different things might define our character. We should accept the change because
change is a natural process. If I find myself in such a situation I first talk to them and tell them about my
background and that it was new to me. I apologize to them if I have hurt them in past and try to adapt to the
change around me. I should be frank and tell everything that I feel and how I am trying to overcome it.

Wellbeing is the state of being comfortable, healthy and happy. Due to his physical disabilities, Harry may
be in emotional and physical pain. Physical disabilities can impact overall wellbeing. Harry might have
physical problems and psychological problems. He may feel sad and bad for himself. He should be
motivated and cheered up for life. He may lose hope for the life that needs to be restored. We should help
him to do his daily life work and make him feel good. He may be struggling with this mental strength. He
may have anxiety, lack of engagement, have bad feelings and negative thoughts. He may try to self-harm
also. He needs proper care and his hopes should be restored.

Harry's physical status can be measured by his movement in daily life. If he is feeling trouble walking or
doing a different activity then we should consider that he is having some sort of problem. we can measure
and observe his daily activity. we can engage him in some sort of exercise and note his reaction and
progress to that exercise. To recover fast he needs to do more exercise which will improve his movement
and make him flexible. We can observe small activities like bathing which can indicate to us about his

recover. So it is important to involve Harry in the types of activities he is going to be involved in.
Harry has all his rights and final decision in such kinds of activities because some activities can cause him
pain and difficulties. activities that we force him to do may cause some accidents or demotivate him so we
should be very careful and make him involved in every decision-making process which affects him. If he is
involved in such a process then he will be motivated to do such activities because he has already
committed to it.

After two months in the residence, you notice that Harry is not relying on his walking frame anymore for
short walks to the bathroom or the dining room. His muscles seem to be getting stronger and he is very
pleased with himself when he doesn’t rely on his walking frame.

8. How will you assist Harry to be as independent as possible while ensuring he uses available support
when it is necessary?

If he is recovering and becoming independent then we should encourage him to do those activities that
can make him feel special and make him independent. We should provide him assistance when he asks or
when he is in need otherwise we should give him freedom and let him do his activities.
We should show his progress and motivate him and make him feel that he can achieve anything he wants.
We should show him an example that can motivate him and become more independent. We should talk to
him and give him what he needs to become independent. We should assist him in his every success and
encourage him.

9. What will you do with your observations of Harry’s improved physical strength and his ability to walk
unassisted for short periods.

With my observation of Harry's improvement, I can analyse when will he become fully independent and in
which sector he needs more attention. He could be set as an example for other clients, how they can
improve themselves. By the data of observations, we can find the time when will he get fully recovered. I
can share that observation with Harry and motivate him. If it goes like that then he can get recovered in a
short period of time. When he uses less assistance than that assistant can be used in other sectors where it
is important

Harry has just started leaving the residence once per month for a day to visit his daughter and her husband
on her farm. They come and pick him up and then bring him back after the day is over. You notice after one
of these trips that he has bruises all over his upper arms. You ask him how he got the bruises and he just
shrugs it off and says it’s nothing, he hasn’t even noticed them. Harry is rather melancholy but you can see
he is upset and he keeps rubbing the bruises.

10. What will you do in this situation and why? Use your organisation’s policies and procedures in this case to

At first, we should try to find the truth about the bruises. We need to make Harry share his feeling and make
an environment where he can open himself. If he didn't talk or open himself then we should talk about that
situation to his daughter and husband. We should give him medication and make him feel comfortable. We
should warn his family what can be the consequences if the problem arises again and again. If the situation
is bad then we should work according to the policy of the organisation. Organisational procedures that

Billy is 25 years old and is very overweight. Because of this, mobility is difficult, his breathing is laboured and
the doctor is concerned about his heart. Billy has an intellectual disability which makes communication
difficult. Billy’s doctor has asked that he be given a diet and exercise program. He lives at home with his
mother but he comes to your facility on a part-time basis.

1. Write a goal for Billy’s individual diet and exercise plan in relation to his diet and nutrition.

The main goal for Billy's individual diet and exercise is to make his heart function well and loss weight. He is
young so his diet should be protein-based and fibrous like fruits and green vegetables. We can plan
exercises like brisk walking, swimming, cycling, jumping rope etc. the rate of exercise should be low at the
beginning and it should be slowly increased. Diet and exercise should be balanced

walk only. He should be encouraged to walk or move from one place to another place without any help. We
should motivate him to exercise and we should exercise him to encourage him. He should not be
dependent on those tools for every activity. He should try to do some work by himself without the help of
the tools. Slowly the use of aid should be decreased and make him independent to those tools

The assistance that can encourage Billy to know his strength and achievement through the program can
be his progress report for a few weeks. He can feel the mobility of his body and can be motivated to work
more. To make it more successful he can feel the change within himself. We can show him as an example
to others which can encourage him to work more and be an example for others. We can assist him while
doing exercise and we can plan his diet. We can change the frequency and intensity of his exercise with the
progress he is achieving. His diet can be improved according to his progress. We can assist him to achieve
his goals and motivate him in different ways

out of the pantry and fridge. She tells you he often becomes angry if she denies him junk food, so she has
been giving in to him.

10. Explain how you will follow your organisation’s procedures based on what you have observed during your
time with Billy and what his mother has said. In your answer, you will also need to explain why your
actions are important. Write down the name of the procedures you would follow.

At first, we should encourage Billy not to do such an activity. We should educate him on what can be the
consequence of his habit. He should be kept in anger management class and be motivated to engage in
activity that encourages him. If his mother can’t control him at home then he should be admitted to the care
home for a certain period of time. If he gets admitted to the care centre then his activity can be observed
and act according to it. We can use different medications which can help to tackle his hunger. We can put
him in observation and make a plan according to it. We should slowly reduce junk food. In the place of junk
food, we can replace it with other food which is healthy and test like same. He can join a mediation class
where he can learn how to control his desire for junk food. If the case is serious then we can make a routine
where we can give him junk food but little and slowly decrease it. We should consult a doctor and
psychiatrist if needed. He should be encouraged by his support group and make him fell special

Billy’s mother tells you that Billy has been walking awkwardly and she has seen him rubbing his feet and

expert on that topic who can help him to get rid of it. As being his caretaker it is my responsibility to address
the problem as soon as possible. At first, I should have all the information about the problem then only I can
refer others. If the problem is big then I should consult a doctor and change his exercise and diet plan. To
make him comfortable we can stop doing heavy exercise and move to small and light exercise. He should
not be standing more because his leg might have hurt due to being overweight and exercising. Medicine
on the recommendation of a doctor should be used. With this information, I should change the exercise
routine and make other diet plans which solve this problem

INSTRUCTIONS:
For this task, you will participate in a role play to demonstrate that you can support independence and
wellbeing. Your assessor will play the role of Cathy, and you will play the role of a leisure and lifestyle assistant.

You are a leisure and lifestyle assistant at a disability support centre. One of the people you provide care for
is a 19-year-old named Cathy. Cathy has a spinal cord injury and is paralysed from the waist down so she
makes use of a powered wheelchair. Before Cathy had her accident, she loved dancing and she was
involved in dancing competitions.
Immediately after Cathy’s accident one year ago, she suffered terrible depression at the thought of never
being able to walk, or dance, again. She lives at home with her parents but she visits the disability centre
twice per week in the afternoons to socialise with other people in a similar situation to her own, and to
engage in activities.
Cathy enjoys most of the activities she participates in at the centre. She is not so fond of the arts and crafts
activities on a Wednesday, but this is the only class offered during the time on Wednesday she visits the
centre. She especially loves the physical activities which are tailored to upper body fitness and ball sports,
however, she only attends one of these classes per week and the centre runs three. Recently however, she
has confided in you that she would love to find out about wheelchair dancing as a sport. She has been
struggling to adapt to her new life since the accident and has been struggling with her depression on and
off. The fact that she is not playing competitive sports anymore affects her greatly.

Do some research so you can provide some information to Cathy about where she can participate in
wheelchair dancing in your local area (use the nearest city location if you are in a rural area) and have it ready
to bring to the role play.
Cathy has asked you for help in accessing information about wheelchair dancing and will need to take some
form of information with you like an iPad, laptop, brochures or printed information. You are going to sit in the
lounge of the centre and discuss options for her together.
Your assessor will provide you with a date and time for the roleplay. Meet with your assessor who will play the
role of Cathy, you will play the role of the leisure and lifestyle assistant. You need to ensure the environment is
suitable for Cathy and her wheelchair and deal with any potential hazards before the time.
