We are respectfully submitting for consideration and approval of the General

Manager the Time Extension No. 1 for the contract: Kapyaan Heights Located at
Brgy. Balabag, Digos City, Davao del Sur

Hereunder are pertinent data and information, to wit:

I. Contractual Data:

II. Justification

The contractor requested for Contract Time Extension No. 1 due to unworkable days
caused by the inclement weather condition on site and the issuance of Work
Suspension Order No. 1 by reason of inability to secure the following permits and
clearances of the project, to wit:

1. Land Reclassification and DAR conversion clearance certificate;

2. Preliminary Approval and Locational Clearance (PALC); and
3. Development Permit (DP).

E. Recommendation

In view of the foregoing, we are recommending for the approval of Contract Time
Extension No. 1 due to the above-cited reasons, thereby adjusting the contract
duration from Seven Hundred Twenty (720) Calendar Days to Eight Hundred Forty-
Nine (849) and revising target completion date from August 17, 2021 to December
13, 2023.
