DIVISION MEMORANDUM
1. This Order provides for the Policy and Guidelines on the Proper Distribution, Care,
Recording, Retrieval and Disposals of Tablets issued by Schools Division Office in order
to maximize their use and eliminate or minimize damages and/or losses. lt also provides
the guidelines on the proper recording and disposal of damaged and condemned lablets.
The tablets are provided lo students for free and are intended to be used for at least five
(5) years. lt is the responsibility of every student, teacher and school head to exert all
efforts to.protect the tablets from damage or loss.
3. All concerned personnel are enjoined to strictly implement and adhere to the enclosed
guidelines and procedures. Please see the atiaehed Enclosures for your reference'

ENCLOSURE NO. 1
. At the start of the school year, the school supply officer/designated property
custodian shall be responsible for issuing the Tablets to classroom
teachers/advisers, who shall likewise issue the same to each parenulearner.
. The Schools Division Superintendenl and School Heads should ensure that teachers
accept Tablets issued to them by the school supply officer/designed property
custodian for the use of the pupils. The teacher-advisers who are apprehensive to
accept the Tablets due to penalties of losses and accountability, shall be dealt with
administratively.
. Tablets issued to each parenUstudent shall bear the property code number, where,
the Tablets identify with the user. The supply officers/designated property custodians
shall maintain a record/logbook of Tablets issued to each user. The property
custodians may facilitate the recording by having each student sign in the
record/logbook against the designated property code number for which they were
assigned a Tabtot.
. Teachers are encouraged to read to the parents "Agreement Form" to be signed by
the parent and school. This shall minimize damage on Tablets due to improper
handling and usage.
' . Students have no ownership, interest or title in the issued tablets
. The tablets are provided to students for free and are intended to be used for at least
five (5) years.
e The School shall form a help desk who will provide technical assistance to
parents/students in case of damaged or inoperative tablets.
. The Parentsl Guardian shall take full responsibility in taking care of the devices
following the conditions stated in the issuance documents and in the bonoweCs card.
. The recipient l6ern6r must attend a mandatory training conducted by the School ICT
Coordinator on the use of th6 tablets which shall includo prop6r use, case, cleaning
and storage as well as avoiding theft. Training is prerequisite to borrowing the tablet
. The School shall provide proper and safe storage for the tablets after its retrieval
Responsibilities of Students
o Handle the issued tablet with care, protecting its functionality and appearance

. The tablet will be used only for educational purposes. lt may not be utilized for
personal profit or non-profit purposes such as advertising, rentals or such other usee
r Must not install software onto th6 tablet unless authorized to do so by the School

r Must not modify, upgrade or attempt to rcpair the teblet without the express
permi6sion of the school
o Must treat the tablet in a similar manner as other schooFowned educational tools
such as textbooks
Damaged Tablet
. If the student discovers that his/her tablet is damaged to the point that it is non-
functional or operates with reduced function, the student shall notify the teacher-
advisers immediately.
. The Help Desk will assess the damage on the tablet and provide necessary technical
assistance.
. lf it is determined that the damage is a caused by the negligence of the students, the
cost of the repair or replacement will be paid by the parent subject to the existing
laws and regulations
o lf it is determined that the damage is without the negligence on the part of the
student, the expenses necessary for the repair or replacement of the tablet will be
charged to the school MOOE subject to regular auditing rules and regulation.
LosUStolen Tablet
r ln the event that the tablet is losUstolen, the student should notify his/her teacher-
advisers immediately.
. ln case of loss, the parents shall submit a notarized Affidavit of Loss while for theft, a
copy of Police Report.
o lf it is determined that after the investigation the lost was due to the negligence or
fault of the student, the parents shall pay the value of the tablet.
r lf the lost was not due to the fault of the learner, the teacher must submit a report to
the Property Custodian to support his/her application for Relief from Accountability.
Tablet Retrleval
. The student shall return the issued tablet at the end of the school year or the date
announced by the School
r The School head should clo$ely monitor retrieval procedures and involve the whole
community in reminding the students to return the tablets before the end of the
school year
o The School will inspect the tablet for damage and will perform general mainlenance
and upgrade. The. School shall also report to the Division Office the implementation
of the project during the current school year.

Tablet Disposal
. The school property custodian should identify obsolete, damaged, or worn out
Tablets and submit the inventory to the Division Office.
. The principal/school head should ensure that all Tablets for condemnation/disposal
are no longer in the inventory list.
. The Division Office in coordination with the Disposal Commiltee shall notify the
schools on the disposal dates.

LEART{ER/PARENT AGREEMENT
This agreoment is made between (Student Nam6), (Perenucuardian Name), and the (Name
of School). The equipment referenced in this agreement, Tablet and its accessories.
By signing this form, I understand that I am borrowing this tablet and its accessories.
By signing this form, lam also stating that I have read and completely understood the
Acceptable Use Policy and Tablet Agreement form which states that I will be responsible for
theft of or damage to this tablet while it is signed out to me and further that I will return this
tablet once no longer enrolled in (Name of School). The term of this agreement shatl be for
the current school year only.

tungkuling nararapat kong tuparin sa paghiram ng tablet ng aking anak o batang
pinangangalagaan.
Naiintindihan kong gagawin ng paaralan ang bahagi nitong bigyan ng magandang edukasyon
at proteksyunan ang aking anak o batang pinangangalagaan kung kaya sila ay nararapat
sumunod sa makataong disiplinang ipinapatupad nito sa taong pampanuruan 2O2O-2O21.
