By signing this form, I am also stating that I have read and completely understood the Acceptable Use Policy and Tablet
Agreement form which states that I will be responsible in case of loss or dam age to this tablet while it is signed out to me
and further that I will return this tablet once no longer enrolled in Narra National high School. The term of this agreement
shall be for the current school year only.

I also understand that if I participate in the said agreement, it is my responsibility to pick up and return the device for the
Narra National High School-Matatag 2, Panacan 2, Narra, Palawan. In a timely manner and that this device is intended
only for the current school year.

PANGANGALAGA AT PAGPAPANATILI NG KAAYUSAN NG TABLET

● Ang mag-aaral/magulang ang responsable sa pang-araw-araw na pangangalaga at paglilinis ng tablet.

Inaasahang pag-iingatan at pangangalagaan ng mag-aaral ang paggamit ang naturang tablet.
● Ang tablet ay hiram lamang. Ito ay pag-aari ng gobyerno at kailangang isauli sa katapusan ng taong
pampanuruan 2020-2021.
● Anumang sira o pagkawala ay kailangang ipabatid agad sa paaralan sa loob ng 24 oras. Kung ang pagkasira o
pagkawala ay dahil sa kapabayaan ng mga mag-aaral, nang mga magulang ay magbabayad ng pagpapagawa o
pagpalit ng bagong tablet.
● Panatilihinng itabi ang tablet sa isang ligtas na lugar kung hindi ginagamit.
● Kung may sira ang tablet huwag itong buksan o galawin ang mga piyesa nito nang walang pahintulot ng
paaralan.
● ang tablet ay para lamang sa “remote learning activities.” Bawal magdownload ng apps, pictures at videos na
walang kinalaman sa modular distance learning.”

PAGTANGGAP NG MAGULANG AT MAG-AARAL

Ang aking paglagda ay nangangahulugang lubos kong naunawaan ang lahat ng mga tungkuling
nararapat kong tuparin sa paghiram ng tablet ng aking anak o batang pinangangalagaan.

Naiintindihan kong gagawin ng paaralan ang bahagi nitong bigyan ng magandang edukasyon at
proteksyunan ang aking anak o batang pinangangalagaan kung kaya sila ay nararapat sumunod sa makataong
disiplinang ipinapatupad nito sa taong pampanuruan 2020-2021.

RESPONSIBILITIES OF STUDENTS/POLICY/ GUIDELINES ON THE DAMAGED,

LOST/STOLEN,PROPER RETRIEVAL AND DISPOSAL OF TABLETS ISSUED BY THE SCHOOLS
DIVISION OFFICE

RESPONSIBILITIES OF STUDENTS
● Handle the issued tablet with care, protecting its functionality and appearance.
● The tablet will be used only for educational purposes. It may not be utilized for personal profit or non-profit
purposes such as advertising, rentals or such other uses.
● Must not install software the tablet unless authorized to do so by the School Principal/ICT Coordinator.
● Must not modify, upgrade or attempt to repair the tablet without the express permission of the school
● Must treat the tablet in a similar manner as other school-owned educational tools such as textbooks

DAMAGED TABLET
● If the student discovers that his/her tablet is damaged to the point that it is non-functional or operates with
reduced function, the student shall notify the teacher -advisers immediately.
● The Help Desk will assess the damage on the tablet and provide necessary technical assistance.
● If it is determined that the damage is a caused by the negligence of the students, the cost of the repair or
replacement will be paid by the parent subject to the existing laws and regulations
● If it is determined that the damage is without the negligence ont he part of the student, the expenses necessary
for the repair or replacement of the tablet will be charged to the school MOOE subject to regular auditing rules and
regulation.

LOST/STOLEN TABLET
● In the event that the tablet is lost/stolen, the student should notify his/her teacher-advisers immediately.
●In case of loss, the parents shall submit a notarized Affidavit of Loss while for theft, a copy of Police Report.
● If it is determined that after the investigation the loss was due to the negligence or fault of the student, the
parents shall pay the value of the tablet.
● If the loss was not due to the fault of the learner, the teacher/adviser must submit a report to the property
custodian to support his/her application for relief from accountability.

TABLET RETRIEVAL
● The student shall return the issued tablet at the end of the school year or the date announced by the school.
● The school head should closely monitor retrieval procedures and involve the whole community in reminding the
students to return the tablets before the end of the school year.
● The school will inspect the tablet for damage and will perform general maintenance and upgrade. The school
shall also report to the Division Office the implementation of the project during the current school year.

TABLET DISPOSAL
●The school property custodian should identify obsolete, damaged, or worn out tablets and submit the inventory to the
Division Office.
● The Principal/School Head should insure that all tablets for condemnation/disposal are no longer in the inventory list.
● The Division Office in coordination with the disposal community shall notify the schools on the disposal dates.

b) Naipaliwanag na sa akin ng eskwelahan na ang nasabing tablet ay PAGMAMAY-ARI NG PAARALAN at

IPINAHIRAM lamang upang magamit sa pag-aaral ng aking anak o batang nasa aking pangangalaga;
c) Iingatan at pangangalagaan ko ang nasabing pang-edukatibong tablet para sa “distance learning” ng nasabing
mag-aaral para sa School Year 2020-2021. Sisiguraduhin ko na ang nasabing tablet ay itutuon lamang sa pang
edukatibong gamit at hinding hindi ito ipapahiram o papaupahan sa ibang tao;
d) Ang tablet ay hindi I-installan ng mga games o apps na walang pahintulot ng DepEd o SDO Palawan at hindi
ito gagamitin para maka log-in at maka-gamit ng Facebook, Messenger, YouTube, TikTok, Instagram,
Twitter at iba pang application na walang kinalaman sa edukasyon ng bata at walang pahintulot ng DepEd
o SDO Palawan;
e) Aking IPAPAAYOS AT/O PAPALITAN ang NASIRA, NAWASAK o NAWALANG pang-edukatibong tablet
bilang resulta ng paggamit ng nasabing mag-aaral o dahil sa kapabayaan ko, ng nasabing mag-aaral o ng
ibang tao, o dahil sa hindiinaasahang pangyayari at suliranin ayon sa Article 1942, Civil code of the
Philippines;
f) Dadalhin at ipapakita, matapos makatanggap ng paunawa sa school, ang nasabing pang-edukatibong tablet sa
nararapat na Opisyales ng Paaralan para sa inspeksyon;
g) Naipalam na sa akin na ang DepEd , SDO Palawan, o ang Paaralan at ang mga opisyal nito ay mayroong
karapatang bawiin ang nasabing tablet kung, matapos ang inspeksyon ng opisyal o guro ng Paaralan, ay
nakitaan ito ng mga application na hindi otorisado ng DepEd, SDO Palawan, o ng Paaralan, gaya ng, Facebook,
Messenger, YouTube, TikTok, Instagram, o Twitter ;at
h) Ibabalik ang pang-edukatibong tablet sa nararapat na Opisyales ng Paaralan sa katapusan School Year2020-
2021 o kapag ito ay hiniling na kunin ng Paaralan pagkat ang sitwasyon sa lokalidad ay papahintulutan na ang
harapang klase na naayon sa mga patnubay na itinalaga ng Pambansa o Lokal na gobyerno na nakakasakop sa
School o sa anumang kadahilanan ay maituturing na makatwiran ng School o ng SDO Palawan.

3. Na aking ginawa itong pagpapatunay at pagpapatibay upang patutuhanan ang mga nabanggit sa itaas at bilang pagsunod sa
mga kinakailangan na itinalaga ng SDO Palawan upang ang nabanggit na pang-edukatibong tablet ay maibigay upang magamit
na nabanggit na mag-aaral sa “distance learning” para sa School Year 2020-2021.
