Dear ______________________________,
I am writing to inform you about your child's recent inappropriate behavior in school. As you are aware, our
school has a strict code of conduct that all students are expected to follow, and unfortunately, your child has
violated these rules.

The purpose of this letter is to notify you of your child's behavior and to request your cooperation in
addressing this matter. We believe that a collaborative effort between the school and parents is essential in
ensuring that our students are well-behaved and respectful towards others.

We understand that your child's behavior may be due to various reasons, and we would like to work with
you in finding a solution to this issue. We encourage you to speak with your child about the severity of their
actions and the potential consequences that come with them.

To that end, we would like to propose an agreement between the school and the parents to ensure that
your child's behavior is corrected and does not reoccur. This agreement may include but is not limited to:

1. Meeting with school staff to discuss your child's behavior and create an action plan to improve their
conduct.

2. Regular check-ins with the school to monitor your child's progress and behavior.

3. Encouraging your child to attend counseling or other support services that may be available to them.

4. Providing consequences at home that align with those set forth by the school.

We hope that you will take this matter seriously and work with us in addressing this issue. We are committed
to ensuring that all of our students are safe and respected in our school environment.

If you have any questions or concerns, please do not hesitate to contact us.

Sincerely,

Charede Luna Bantilan Received:
