MEMORANDUM OF AGREEMENT
This Memoradum of Agreement, represented by the School Head, Janet V. Cabiente, Principal II,
with school address at Zone 3, Panganiban, Tayug, Pangasinan, herein after referred to as SCHOOL.

a resident of _____________________________________________________, herein after referred to as

LEARNER.

WITNESSETH that:

WHEREAS, LEARNER is currently/officially enrolled as Grade _____ of the SCHOOL for the School
Year 2022-2023.

WHEREAS, a Coby mini-laptop is given by the DepEd for its DepEd Computerization Program;

WHEREAS, with regard to the use and maintenance of the distributed laptop/tablet, the following shall be
considered:

1. The principal/school head upon receipt of the devices which are government properties, shall
ensure that the devices are functional, no damages, properly used for, cared for, safeguarded from
natural and human elements, and recorded upon turnover to the LEARNERS;
2. The LEARNER, in turn, can bring home the device to be used for educational purposes only.
3. The LEARNER is liable for any losses or damages while the device is in his/her family custody
especially if it is determined that there was negligence and misuse of the laptop/tablet by the
learner’
4. The LEARNER shall immediately inform the SCHOOL, in case the laptop/tablet is broken.
LEARNER may personally have it repaired in any computer repair service center;
5. The LEARNER is required to return the laptop/tablet intact and with no damages, upon the
closing of the school year. A LEARNER who has lost his/her tablet due to natural causes or force
majeure must write an Affidavit of Loss/Letter, signed by the learner and parent/guardian, stating
about the loss and explaining the circumstances regarding the said loss within five (5) days from
the occurrence of loss.

High School, Tayug, Pangasinan.
