MINUTES OF THE GRADE 9 EINSTEIN HOMEROOM PTA MEETING OF BATARAZA NATIONAL HIGH SCHOOL, BATARAZA
PALAWAN, HELD ON SEPTEMBER 14, 2021, AT 9 EINSTEIN ROOM
I. CALL TO ORDER
The meeting was called to order at about 8:30 in the morning by Ma’am Arosalie R. Claveria, Grade 9 Einstein adviser, followed by an
opening prayer led by Mrs. Domine, one of the parents.
II. ATTENDANCE
Filling up attendance was done. Thirty-one parents were present.

ORIENTATION OF PARENTS
a. Learning Modality
 Parents were informed about the learning modality of the school. The school’s learning modalities were modular distance learning
through printed learning materials and radio-based instructions. The modality is known as "blended.
 The learners will be given printed learning materials in the form of handouts and worksheets that will be used by them during radio
lessons.
b. Class schedule, schedule of release and retrieval
 The parents were informed about the schedule of classes for the learners. They were also given a copy of the class schedule with the
teacher’s contact number in case they have questions or clarifications pertaining to the lesson for each subject.

 They were also informed that the schedule for the release and retrieval of worksheets and handouts was every Friday morning.
 Parents/guardians shall be the ones to claim the set of handouts and worksheets and shall be the ones to submit the accomplished
worksheets of their children on the said schedule.
 Baskets were prepared by the adviser where they could easily drop all the worksheets.
c. Feedback slip
 Parents were also informed about the importance of completing the feedback slip. That they should advise their children to complete
the feedback form in order for the teacher to understand their concerns.
d. Agreement form
 The parents were given an agreement form for the school year 2021–2022.
 The agreement includes the roles and responsibilities of the parents and teachers towards the learners who intend to help each other in
order to avoid them dropping from the school. They are just going to guide their children in accomplishing the tasks, but they will not
be the ones to write the answers on the worksheets.
 Also, the adviser added and gave emphasis that siblings, brothers, parents, and guardians are not allowed to accomplish all the
learning activities of their students. If, for example, they were caught completing all of their student's learning activities, the

teacher would return the worksheet and assign them another worksheet to complete, and they would be required to sign a
promise not to do it again in all subjects through a promisory letter.
 All the parents also agreed with the agreement.
e. RELEASE OF FIRST WEEK WORKSHEETS
 All parents were given a set of worksheets and handouts for their children.
f. OTHER MATTERS
 The advisor also announced that they should keep the phones open during school days in order for them to be reached by all the
subject teachers if there are any concerns related to their students.
IV. ADJOURNMENT
 Ma’am Arosalie R. Claveria, declared the meeting adjourned at around 11:30 in the morning.
