STATEMENT OF CONFIDENTIALITY

The Schools Division of Quezon through the Budget Office in cooperation with the SGOD –
Planning and Research Section provides limited, strict access to the Google Drive containing details
on 2022 School Maintenance and Other Operating Expenses (MOOE) fund for Elementary Schools,
Junior High Schools, and Senior High Schools, provided for the following:
1) To be used as reference in the implementation of DepEd Order No. 44, s. 2015; and
2) To inform the School Heads without Financial Staff (Non-IUs) of their Annual MOOE
Allocation, as per DepEd Order No. 008, s. 2019

In connection with the AIP/WFP preparation of said schools, I ______________________,

School Head in-Charge of Governance and Operations (SHiGO) of _______________ DISTRICT
hereby willfully commit to keep any data contained in the Drive confidential, which may otherwise
jeopardize the provision/s of the Data Privacy Act of 2012. I acknowledge and accept the following
conditions, where:
a) Only the SHiGO shall have direct access to the Google Drive containing MOOE details;
b) The SHiGO shall not download the MS Excel files in the Drive but can view needed
information;
c) The SHiGO shall not provide access to Drive files to anybody to secure privacy and
prevent unintended use; and
d) The SHiGO shall use the information/data contained in the Drive for the stipulated purpose
only.

I understand that I am held accountable for the unintended leak of confidential information, and
that breach of this Statement of Confidentiality has administrative and legal implications.
