INSTRUCTION

The purpose of this questionnaire is to help teachers, school counsellors,


psychologists, and social workers understand young people better, so that they
could more effectively plan lessons and services for young people. To do this, we
need your help to provide some information about you.
This questionnaire is confidential and will NOT be read by anyone
connected with your school or by your parents.
All analysis is carried out at the Department of Social Work, National
University of Singapore.
