 Who noticed the problem?  Any leakage around the machine?
 When first noticed?  Who performs (daily) maintenance?
 Where was the problem first noticed?  Weather when problem noticed?
 How much or severe is the problem?  Other machine troubles previously?
 What could you see/ hear/ feel/ smell?  Repair before problem first occurred?
 Once, repeatedly or continuously?  Repair after problem first occurred?
 Last time machine worked normally?  How is the machine being used?
 Pattern or trend?  Was machine operated correctly?
 When does problem not occur?  Anything else we need to know?
 More machines with same problem?  Special circumstances?
 What job done when trouble noticed?  Beware of assumptions
 Who else operated the machine?
 Is this a machine problem or is operator unhappy
with this model or machine size? What-Who-When-Where-How much
