AFFIDAVIT OF SELF-ADJUDICATION

I, IMELDA EDANOL SANTIAGO, of legal age, Filipino and a resident of 577 Ramon Jose St.,
Sta. Maria 1, Balibago, Angeles City, after having been duly sworn in accordance with the law,
hereby depose and state:

1. That I am the only, surviving legal heir of RAULI OLAVI PITKANEN who died in Brgy. Amsic,
Angeles City on June 7, 2020 with last will and testament executed in Darwin, Northern
Territory, Australia. A copy of the Death Certificate and Last Will and Testament of Rauli
Olavi Pitkanen if attached hereto as “ANNEX A and B” consecutively and made and integral
part hereof.;
2. That to the best of my knowledge and information, my partner left no debts or obligations
of any kind;
3. That the said deceased left savings account deposits in the Metropolitan Bank and Trust
Company – Angeles Balibago Branch under Savings Account No.: 374-3374190351 with
account balance of PHP 4,484.40 AND 374-3374107390 with account balance of PHP
1,122,294.67;
4. That as the surviving legal heir, I hereby adjudicate unto myself extrajudicially the aforesaid
savings deposits and I hereby request the Metropolitan Bank and Trust Company to release
to me the entire proceeds of the said deposit;
5. For and in consideration of the said release, I hereby expressly and absolutely remise,
release and forever discharge the Metropolitan Bank and Trust Company, its administrators
and assigns and/or any of its officers or employees from any and all claims, suits of action
against the said bank and I hereby undertake to indemnify the said bank, its officers and
assigns for any loss or damage which they may sustain arising out or in connection with the
release to me of the aforesaid deposit account.
