1.Are you left handed, right handed or ambidextrous (can use both right and left)?
2. Can you describe the type of pain you are currently experiencing?

P: How frequently do you experience this pain?

3. When did the pain start? What are you doing to aggravate and relieving your symptoms?
4. Do you have any experience trauma or accidents?

5. Where is the specific location of the pain in your hand? Can you point to the area in your hand
that is most painful? Kindly rate the pain into 10 is highest and 0 is no pain

6. What time of the day do you usually experience the worst pain?
P: How long does it last?

7. Can you describe your work area? Type of chair (with arm rest or not),

height of the table in relation to the arm rest or when elbow is flexed in a
sitting position, placing of the computer, and devices used (type of mouse).

8. What is your occupation?

P: What are the usual activities in your work that require the use of a hand? How many hours
your working time?

9. Do you have any other present health diagnoses?

P: If so, kindly indicate.

10. What do you want to achieve or what is your goal here in tele-rehabilitation?
Problem List/s
1. Intermittent sharp, shooting pain tingling sensation (PS7/10) on the (R) hand
2. LOM on ® hand towards wrist flexion and extension
3. Decreased grip strength on ® hand

➢ Phalen’s test
➢ Reverse Phalen’s test
➢ Froment’s sign
1. ROM using Goniometer application
2. Grip strength Analysis (Blood Pressure Apparatus)
Virtual Plan of Care
1. Median Nerve gliding exercise x 30SH x 5 reps x 2-3 sets on ® hand
2. Wrist Flexion and Extension Stretch x 15SH x 5 reps x 2-3 sets on ® hand
3. Grip strengthening exercises hold for 3-5 second x 10 times x 2-3 sets
