I am Maria Lita G. Balasta, a registered midwife, I am interested to apply and render services
as Nursing Attendant to be assigned in Gamay, District Hospital.

As to my qualifications, I finished my midwifery course at the Holy Infant College, Tacloban

City and fortunately passed the midwifery licensure examination held April 1997 with an
average rating of 79.10%. I have earned units in Bachelor of Science in Nursing at University of
Eastern Philippines. I have rendered services at the Gamay District Hospital as Nursing
Attendant for 5 years as my work experience.
