SANGGUNIANG KABATAAN OF BARANGAY CALLUZA AUTHORIZED SK

CHAIRPERSON ADRIAN PAUL R. LLANES TO REPRESENT THE SK OFFICE OF
BARANGAY CALLUZA IN THE PORCESSING OF APPLICATION OF TAX
IDENTIFICATION NUMBER (TIN) AND OTHER DOCUMENTS.

WHEREAS, the SK Council of Barangay Calluza, an association existing as a political body of

Republic Act 10742 with the principal place of business at Barangay Calluza, Piddig, Ilocos
Norte.

WHEREAS, in a meeting held on the a forementioned principal place of the business last
SEPTEMBER 1,2021, it was agreed and unanimously approved by the SK Council that SK
Chairperson ADRIAN PAUL R. LLANES be hereby authorized to represent the SK Office of
Barangay Calluza, Piddig, Ilocos Norte in the processing of the application of TAX
IDENTIFICATION NUMBER and other official documents related to the foregoing;

WHEREAS, the SK Council further agreed that such authority granted to SK Chairperson
ADRIAN PAUL R. LLANES include, among others, to sign and execute documents, to process
the application for aforementioned documents;

NOW, THEREFORE, BE IT RESOLVED, AS IT IS RESOLVED BY THE

SANGGNIANG KABATAAN OF BARANGAY CALLUZA to authorize SK Chairperson
ADRIAN PAUL R. LLANES as the representative to the BIR for the application of TAX
IDENTIFICATION NUMBER (TIN) and such other documents as may be necessary.

RESOLVED FURTHER, to furnish copies of this resolution to all concerned government

offices/agencies for their information and appropriate action;

APPROVED this 1st day of SEPTEMBER 2021 at Barangay Calluza, Piddig, Ilocos Norte.
