AFFIDAVIT OF ADMISSION OF PATERNITY

I, ROEL S. ORDANIZA of legal age, Filipino, and a resident of Opol, Misamis

Oriental, after having been sworn to an oath in accordance with law, hereby state that:

1. I am the biological father of RJ NAMOC ORDANIZA who was born on

September 27, 2020 in Malingin, Igpit, Opol, Misamis Oriental;

2. The child’s mother is JENNEFER SUAZO NAMOC;

3. At the time of the birth of said child, I was not yet legally married to her
biological mother;

4. I hereby admit under oath the paternity of RJ NAMOC ORDANIZA for

purposes of allowing the child to use my surname and give the child the rights
in accordance with Republic Act 9255;

5. I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal purpose this may serve.
