THINGS TO PREPARE AND BRING PRIOR TO AND DURING TAKING

THE FINAL EXAMINATION:

1. Validated School ID – Be reminded that if you do not have a physical ID yet, kindly get it
at Bolton Campus and bring your Form 1. You can wait for it an hour and have your
physical ID.

2. Vaccination Card – You need to make sure that you are fully vaccinated. Kindly consider
your type of vaccine in terms of the number of doses to say that you are fully vaccinated.
If you are not yet vaccinated due to some comorbidities, kindly prepare a Medical
Certificate issued only by the Authorized Municipal/City Health Office. Medical
Certificates are all subject to presentation to the management, so it will be more
convenient to be vaccinated.

3. Scanned Copy of your Parental Undertaking Form - Before dropping it to the assigned
dropbox, kindly scan it first or have a picture of it to present upon entering the room
where you will take your exam.

4. SafeDavaoQR – If you are asked to scan a QR, kindly register and have it safe on your
mobile or attach it to your ID. From the outside Davao City residents, please register
yourself as well in SafeDavao QR (You may click this link: https://profiles.safe-
davao.com/)

5. Yourself – Make sure that you condition your body as healthy as it can as early as now.
Drink vitamins and stay hydrated to avoid getting sick. Other than these, study well and
prepare yourself for the final examination.

GUIDELINES ON SUBMITTING PARENTAL UNDERTAKING FORM:

1. Download the Form – Your subject teacher will upload the soft copy of the Undertaking
Form through LMS. Download and print it. It is okay to have it in black and white or
colored, but you need to ensure that it is size A4.

2. Have it Notarized – After downloading, look for a notary office and have it notarized;
there are PAO in Davao City Sanggunian or Hall of Justice. Please be reminded that you
needed 3 hard copies to be notarized, 1 for you, 1 for the Dean’s Office, and 1 for the
Attorney.

3. Dropping at the University of Mindanao (Matina/Ma-a Campus) – The program will


prepare a box where you can drop your Parental Undertaking Forms located both at
Matina Gate and Ma-a Gate. Kindly inquire to the guard where is the dropbox for
Psychology Program. (Suggestion: Insert your notarized Parental Undertaking Form in a
plastic envelope to avoid the form getting wet or having dirt.)

4. Collection of Parental Undertaking Forms – The Program Head will now collate all the
forms and physically sign them. With these, you need to understand why it is much better
to submit it BEFORE MAY 6, 2022. Office hours are from 7:00 AM – 5:00 PM, and
please avoid submitting it at night or past office hours for your safety.

5. Submission to the Dean’s Office – After the Program Head signed all the forms, they will
now be forwarded to the Dean’s Office. All you have to do now is wait for the
announcements of final exam scheduling from your respective Professors.

THINGS TO REMEMBER AS YOU ENTER THE CAMPUS:

1. Validated School ID
2. Hygiene Kit – Includes alcohol, face masks, and wet/dry tissues.
3. Physical Social Distancing – All students shall observe a 1.5-meter physical distancing.
There are arrows in the hallways/stairs to guide you where to go and the right directions
to your classrooms.
4. Temperature Check – The guard will get your temperature as you enter the Gate (Matina
or Ma-a). Suppose you have 37.5 and above, even after a 5-minute rest. In that case, you
shall not be allowed to take the exam. You will temporarily stay in the assigned Isolation
Room/Area.
5. Bringing of Personal Pens
6. The accomplishment of Health Declaration Form – The Security Guard will give you a
form, and you are discouraged from borrowing pens from other students upon filling it.
7. Bringing of Own Food (Lunch/Snacks/Dinner) – There are no available canteens on the
campus. Please bring your food and consume it in the prescribed holding area/dining
area. If you wish to go outside the campus to eat lunch, please make sure that you go
back right away inside the campus after taking the lunch or dinner. You are discouraged
from staying outside of the campus for long periods. Everyone is reminded to proceed to
their classrooms after having a lunch break for about 30 minutes. The other students who
will also take their lunch can have a space.
