General Instructions and information:
1. A case no. refers to the permanent and unique hospital no. of each and every patient that NMMC has cared for which is verifiable and traceable. Submission of the completed scorecard be after every completion of their assigned rotation 4 major departments and shall be within 3 working days after the last day of rotation. This scorecard shall be required for the issuance of the Certificate of Internship form NMMC. 2. Formative assessment - is to monitor student and provide ongoing feedback to staff and student. 3. Summative assessment – is to evaluate student learning at the end of an instructional unit by comparing it against some standard or benchmark. 4. To ensure accuracy of each case number attended by the intern, the RESIDENT-IN-CHARGE of the case must affirm with his/her initials beside each case number of this form.
