AFFIDAVIT OF ACKNOWLEDGMENT

I, _________________, of legal age, married, Filipino citizen, and with residence

address at _________________, under oath, hereby depose and state that:

1. I am the Attorney-in-Fact of Spouses _________________and _________________,

per Special Power of Attorney executed by them on July 29, 2019 before the
Philippine Embassy of London, herein attached as “Annex A” and is made an
integral part of this Affidavit.

2. As Attorney-in-Fact of the above Spouses, I am authorized to do the

following acts on their behalf, to wit:

i. To offer for sale, sell and come to an agreement as to the purchase price
and thereafter to sin and execute any and all documents and receive the
consideration of the sale of their property located at _________________,
covered by _________________in the Registry of Deeds for Province of
Laguna, Calamba Branch;

ii. To transact business with and make payments to any concerned

government agency and private offices for purposes of securing any and all
clearances, certifications, and permits as may be required or necessary in
connection with the said property; and

iii. To make, sign, execute and acknowledge receipt of any or all documents
relative to the foregoing.

3. I fully acknowledge and conform to the aforesaid duties and responsibilities

given to me.

4. I am executing this Affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve.
