1. Affinity Diagram

Based on the discussion forum on establishing basic survey data, there are currently 30 people
who responded to the survey “What causes high incidents of Covid-19 infections in the Philippines?”
Using an affinity diagram, the group compiled some of the reasons that caused the high infection rate in
the Philippines into four main categories. These categories include political factors, healthcare factors,
financial factors, and lastly public factors. In this diagram, we can observe that the public factor has the
most problems, including lack of coordination in the implementation of preventive measures, people not
following security protocols, an increase in mass gatherings, and lack of public compliance.

The check sheet table above was based on the responses of 30 students from COBQUAL-K39
when asked the question "What causes high incidents of Covid-19 infections in the Philippines?” The
table above shows the summary of the data collected through the survey arranged in a descending order.
Based on the responses and table shown above, the top three (3) causes are poor COVID-19 vaccine
rollout, poor public compliance with prevention measures, and lack of contact tracing with a total
response of 15 (50.00%), 13 (43.44%), and 6 (20.00%) respectively. The first cause is concerned with the
lack of availability of vaccines and the slow vaccine rollout rate in the country. The second cause is
concerned with the people’s lack of discipline and poor compliance with the preventive measures
provided by the government such as no social gatherings, wearing of proper PPEs, and other health
protocols or quarantine restrictions. Lastly, the lack of contract tracing efforts and capabilities is ranked
3rd in causes of surges in COVID-19 within the country.

3. Pareto Chart

The chart above was created using the replies of 30 students from COBQUAL-K39 to the
question "What causes high incidence of Covid-19 infections in the Philippines?". The orange Pareto line
indicates that (15 + 13) / 30 = 93 percent of complaints originate from two out of ten complaint kinds, or
20%. (Poor COVID-19 vaccine rollout and Poor public compliance with prevention measures).
4. Cause and Effect Diagram or Interrelationship (Relations) Diagram

As observed in the diagram above, the high incidents of COVID-19 infections in the Philippines
could be attributed to the lack of concrete plans by the government. It can be noted that the Philippines
was one of the last countries in the world to impose travel restrictions. It still accepted people from other
countries that had high infection rates. This shows the government’s lack of concrete plans to effectively
and efficiently manage the public health crisis while the cases were still under control. Due to the high
transmission rate of the original strain and the fatal lack of timing, the Philippines had experienced what
was considered to be the worst outbreak globally. The political factors or causes have such a tremendous
impact in the healthcare system, the economy, and the Philippine society at large. After all, politics does
cover a wide scope that affects every level of society and could have a spillover effect internationally.
Vaccine rollouts still largely depend on the political will of government leaders and people in every
government level to facilitate it effectively and with urgency. As stated by many health experts all over
the world, vaccines are the way out of the pandemic and every country in the world should have access to
the vaccines in order to properly control the virus and mitigate its effects.
5. 5-Why Technique

Based on the discussion forum, most of the students stated that the high incidents of Covid-19
infections are due to inefficient and insufficient roll out of vaccines by the government. This is because of
the lack of available facilities, medical supplies, and medical equipment to properly administer the
vaccines, and due to the masses not being willing to take the vaccines. This is because the government
had a lack of funds in the countermeasure against the Covid-19 pandemic, and the masses being
uncooperative. The reason for this is due to the lack of economic activities due to the lockdown that was
implemented by the government due to the pandemic, and mismanagement of the funds for Covid-19
counter-measure. And the masses’s fear of side effects that may come with the vaccines. As to why is it
so, it is because some people are uneducated and lack information about vaccines, and the Filipinos voted
for leaders or people in charge that were not capable of handling the responsibilities that came with the
position and power.
