1. You are caring for a 78-year-old patient after the creation of an ileal conduit.

What is the best description of the ileal conduit? Describe the complications
that may follow the placement of an ileal conduit. What assessments do you
commonly perform in the immediate post-operative period?

 Ileal Conduit

- It is composed of a segment of the ileum, a portion of the small intestine.

During ileal conduit procedure, a part of the ileum is separated from the
intestine and one end is closed off, while the other end is taken into the
urostomy and converted to a stoma.

 Ileal conduit maintenance

- It requires a separate approach to care since the fluid is emptied into an

ostomy pouch rather than being filtered into a catheter
- The complications associated with an ill-fitting pouch device result in an
irritated stoma and skin

 Preventing skin and stoma inflammation

- Utilize a pouch device that fits well. Following treatment, the freshly
developed stoma is dimishing. Notify the doctor or ostomy nurse if the
stoma varies in size by more than a half inch in a single day.
- Take care when removing the pouching device
- Unless there is a problem, do not uninstall the pouching device more than
once a day.
- A pouch with an excessively wide opening may leak urine and irritate the
skin

 Complications that occur early

- Associated with the bowel

- Leakage from the ureteroenteric anastomosis
- Necrosis of the conduit
- Fistula enteric

 Stoma Related

- Failure of the kidneys

- Aphrodisiac
- Associated with abdominal wall
- Changes in metabolism

2. Enumerate the Risk Factors for Cervical Cancers.

 Having given birth to three or more children

 Using birth control pills for a long time (five or more years)
 Having several sexual partners
 Having HIV or another condition that makes it hard for your body to fight off
the health problems
 Smoking

3. A 55-year-old patient has been diagnosed with cervical cancer. She reports
that she has a strong family history of cancer; two sisters have breast
cancer. Her mother died of cancer when the patient was a child, and she is
not certain of the type of cancer. Because of her strong family history, she is
concerned about the health status of her twin daughters who are in their
early 30s. She has asked you to discuss the risks for cancer with them.
Explain what counseling and education you will provide to the patient
and her daughters. Identify the evidence base for the counseling and
education and the strength of that evidence. How will you approach your
counseling and education?

 Evidence based counseling

- Maintain a daily workout routine and a balance diet on a timely basis

- Anything that raises a person’s chance of contracting cancer is considered
a risk factor
- Just about 5% to 10% of all cancer cases are hereditary
- Any individual who have a number of risk factors never grow cancer, and
those who have none do.
- Maintain an energizing attitude and a constructive frame of mind at all
times.
