2. This pertains to Violation of RA 9287 wherein one

___________________ y ____________, male, married, 48 years old, born on
July 15, 1974, farmer a resident of Purok 1 ___________________, Bukidnon.

3. Pertaining to the arrest of ___________________ y _________ for

violation of RA 9287 illegal number games or suertres dated September 15,
2022. Subject person was inquest in the above cited case before the office of
Provincial Prosecutor (MARAMAG ANNEX) __________, Bukidnon under docket
number X-01B-INQ-22I-00005 dated September 16, 2022 with administering
officer Hon. ___________________________ Assistant Provincial Prosecutor.
Subject person did not avail of his right to preliminary investigation, therefore, an
information for violation of Republic Act 9287 was filed at 6 th Municipal Circuit
Trial Court, ___________________, ___________________, Bukidnon under
criminal case number 5303-22 dated September 16, 2022. After which, subject
person put up a cash bond in the amount of three Thousand Pesos (3,000) under
official receipt no. 2751762 for his temporary liberty dated September 16, 2022
and was approved by Hon. ___________________ Acting Presiding Judge, 5th
Municipal Circuit Trial Court, ___________________, ___________________,
