EXECUTIVE ORDER NO. 17

SERIES OF 2020

EXECUTIVE ORDER CREATING THE COVID-19 CONTACT

TRACING TEAM OF THE MUNICIPALITY OFKALAYAAN, LAGUNA

WHEREAS, the Department of the Interior and Local Government issued

Memorandum Circular Mo. 2020-73 dated April 13, 2020 outlining the Guidelines for the
Conduct of the Expanded Testing Procedures for COVID-19;

WHEREAS, said MC declares as a policy, the functions of Local Government Units

to fully assist and cooperate with the Department of Health and other health personnel by
setting the necessary platforms and facilities in carrying out the interim guidelines for the
conduct of expanded testing for COVID-19;

WHEREAS, the same issuance directs that all LGUs are advised to increase the
number of trained Contact Tracing Teams;

NOW THEREFORE, I, , Municipal Mayor of

______________, Laguna, by virtue of the powers vested in me by law, do order the
following:

SECTION 1. The (municipality) CONTACT TRACING TEAM (CTT) is

hereby created.

SECTION 2. The CTT shall be composed of the following:

CHAIRPERSON : __________________ Chief of
Police
CO-CHAIRPERSON : __________________ Municipal Health
Officer
MEMBERS : __________________ MPS Staff
__________________ MHO Staff
__________________ Fire Marshall
__________________ MDRRMO
Furthermore, there shall each be a Barangay Health Worker, BHERT Member
and Barangay Public Safety Officer per barangay which shall comprise the Municipal
CTT. The names of these members shall be submitted to the Municipal Health Office

SECTION 3. The CTT shall perform the following functions.

a. Design and implement contact tracing for all persons with known exposure
to a COVID-19 patient to ensure that they are strictly observing the home
quarantine measures;
 b. Monitor the status of persons with Suspected Cases and Possible Cases,
and family members with known contact to a confirmed case to mitigate
possibility of contagion;
c. Regularly report to the (municipality) COVID-19 Task Force;
d. Prepare a Status Report (daily action report) covering 12mm to 12nn of the
following day, following the template/matrix prescribed by DILG MC No.
2020-73;
e. Plan and recommend programs and strategies to rebuild consumer
confidence and reestablish economic activities in the Municipality;
f. Perform other activities incidental and necessary for the accomplishment of
the foregoing , including but not limited to disclosure of such information
and data, provided this is used in conjunction with the provisions of RA
11332 as to the confidentiality of the identity of and information related to
all affected persons.

SECTION 4. This Executive Order shall take effect immediately.
