AN ORDER CREATING THE COVID-19 CONTACT TRACING TEAM OF BARANGAY ___________

DINALUPIHAN, BATAAN

WHEREAS, pursuant to Section 15, article 2 of the 1987 Philippine Constitution and Sec. 16 , the
General Welfare Clause, of the Local Government Code, the local government units are charged with
the promotion of health and safety within jurisdictions. As such they are to take lead in the prevention
and control of corona virus at the local level;

WHEREAS, Republic Act No. 11496, or the “Bayanihan to Heal as One Act”, placed the country in
a State of National Emergency, and has declared the urgent need to mitigate, if not contain, the
transmission of COVID-19;

WHEREAS, Department of the Interior and Local Government (DILG) Memorandum Circular No.
2020-073 provides the Guidelines for the Conduct of the Expanded Testing Procedure for COVID-19,
including the organization of its Contact Tracing Team (CTT);

WHEREAS, Department of the Interior and Local Government (DILG) Memorandum Circular No.
2020-077 directed all Local Government Units (LGUs) to fully establish and activate their respective Local
Task Force Against Covid-19 and align all their actions with the national strategy framework;

WHEREAS, Department of the Interior and Local Government (DILG) Advisory dated July 21,
2020 directed all Barangays to create their own Contact Tracing Team to conduct contact tracing for all
persons with known exposure to COVID-19 patient, ensure they are strictly observing quarantine
protocol and monitor the status of probable, suspected, and confirmed cases to mitigate the spread of
the virus;

WHEREAS, Department of the Interior and Local Government (DILG) Memorandum dated July
30, 2020 provided that in composing the Barangay Contact Tracing Teams, Local Chief Executives may
follow the composition of Barangay Health Emergency Response Team (BHERT) as provided by DILG
Memorandum Circular 2003-95, in addition to other members that the LCE may designate and in
coordination with the city/municipal health officer and/or their respective LESUs.
 NOW, THEREFORE, ____________________, Punong barangay of Barangay ________________,
Municipality of Dinalupihan, Province of Bataan by the powers vested in me by law, do hereby order:

SECTION I. This Order shall be known as the CREATION OF THE BARANGAY ________________
DINALUPIHAN, BATAAN COVID-19 CONTACT TRACING TEAM.

SECTION II. COMPOSITION. Based on the DILG Memorandum dated July 30,2020 with the
subject “Additional Advisory on the Composition of Barangay Contact Tracing Teams”, the Barangay
____________ Dinalupihan, Bataan COVID-19 Contact Tracing team shall be composed of the following:

SECTION III. FUNCTIONS. The Barangay ___________________ Dinalupihan, Bataan COVID-19

Contact Tracing Team shall have the following functions
• Conduct contact tracing for all persons with known exposure to COVID-19 patient;

• Ensure that the persons with known exposure to COVID-19 patient are strictly observing
appropriate home quarantine measures or are properly situated in a government-managed isolation
facility;

• Monitor the status of probable, suspected, and confirmed cases including family members with
known contact to a confirmed case to mitigate the spread of the virus;

• Submit necessary and timely report to other appropriate government agencies; and

• Conduct such other activities that are deemed necessary or related to contact tracing.

SECTION IV. EFFECTIVITY. This Executive Order shall take effect immediately upon signing hereof
and shall remain effective and binding unless revoked, repealed or modified.
