MEMBERSHIP CONTRACT

I, ANGEL KARL S. SERRANO. , with 12025828 of legal age fully and conditionally agree and bind myself to serve DE
LA SALLE UNIVERSITY, INC. as a member of the DLSU Harlequin Theatre Guild, under the auspices of the Culture
and Arts Office (CAO) for the period covering AY 2022-23, subject to the rights and obligations stipulated in CAO’s
Interim Guidelines and my group’s operations manual which I have carefully read and understood, such policies
rules and regulations as are or may be promulgated by the office from time to time and subject further but not
limited to the following:
1. Attendance to all meetings, practices and performances and such other related activities at which my
attendance is required or requested. The trainer and officers shall be duly informed if I am not available, at
least two (2) days before the date.
2. Permission for CAO to secure a copy of the following documents, in consideration of the data privacy law. It
is understood that the details of this information will be deleted one term after I graduate/ resign from the
group:
● my medical clearance (physical and mental health) throughout the academic year, to inform them
of the medical conditions that might affect my performance and participation in the group
● my disciplinary records, and my involvement in other university offices, in the event that I will run
for office as an Executive Board of my CAO group.
● my academic records, for the Arts Quality Assurance that will serve as the basis for the tuition
discount allocation.
3. Attendance to all free online artistic and managerial training given by the Culture and Arts Office, needed for
my personal development.
4. Observance of all safety reminders provided by the CAO trainer in the conduct of the online artistic training.
That if I disregard or fail to follow those instructions or should act on my own, I, together with my parents,
shall have no claim against the school, its officers, faculty/adviser/staff-in-charge/facilitator should any
damage be caused or liability be incurred to property or person.
5. Fulfill any contribution (monetary or in-kind) or sponsorship quota agreed upon by the officers and
members, to be able to carry on the events planned for the year.
6. Safekeeping and return of any property or costumes assigned to me by the group and provide copies of any
soft files left under my custody.
7. Consent for the use of my photographic and film files, for purposes of performance exhibitions in all forms,
media and manner, in conjunction with my name or a fictitious name, for advertising/publicity, trade,
promotion, exhibition, or any other lawful purposes where they may be viewed by the public now and in
the future, regardless of whether that use or matter is known to me; except in cases when my name will not
be given to the press or public without my consent. I also understand that I may cancel this permission in
writing, and that aforementioned party/ies will take all reasonable steps to ensure that the
photograph/video is withdrawn from future use.

It is understood that any violation of the above-mentioned duties and conditions, gives the Culture and Arts Office
the right to immediately terminate my contract or hold my clearance for entry/graduation, by giving me written
notice thereof.
The term of this contract shall be for the period of September 5, 2022 to August 31, 2023, after which this Contract
shall automatically expire. Should I graduate earlier than the expiry date of this contract, I shall only serve the group
until my last committed active term of which I shall duly inform my trainer and our Executive Board.
