This Internship Agreement is made and entered into by and between APRIL JOY
J. MANANGAN (Intern) of legal age residing at Brgy. Ganduz Pantabangan, N.E. and
hereinafter referred to as the FIRST PARTY.
and
The PORAIS NATIONAL HIGH SCHOOL, in San Jose City, the Host Training
Establishment (HTE) with principal address at Porais, San Jose City, N.E. represented
by its School Principal, ALDRIN J. BALOC hereinafter referred to as the SECOND
PARTY.
WITNESSETH:
WHEREAS the First Party and the Second Party are concerned about the
professional training of the future teachers;
WHEREAS the First Party and the Second Party are committed to the completion
of the training program as embodied in the Intern’s Learning Plan;
WHEREAS both Parties believed that the major key in reaching that goal is a
quality pre-service training program that will provide the student-interns opportunities to:
1. Develop the knowledge, skills and desirable attitudes and gain hands-on
experiences in actual teaching;
2. Apply their learning in actual classroom settings;
3. Gain comprehensive and realistic perspective of the teaching-learning
situation and day-to-day school activities; and
4. Reflect on their qualities of commitment vis-a’-vis the demands of the
teaching profession.
WHEREAS both Parties believe that aside from the usual practice teaching,
intensive Field Study, prior to Practice Teaching, is necessary to adequately prepare the
student-interns for their functions and responsibilities.
NOW THEREFORE, premises considered, the Parties agree to the following:
1. The FIRST PARTY will:
1.1 Enter into an internship contract and/or agreement with the participating Host
Training Establishment (HTE);
1.2 Sign all the required documents necessary for his/ her participation in the
internship program;
1.3 Comply with the provisions of the contract/ agreement including the rules and
regulations of the Higher Education Institute (HEI),Host Training
Establishment (HTE) and the Commission of Higher Education (CHED) at all
times;
1.4 Undergo the required orientation program conducted by the HEI and the
HTE;
1.5 Report for internship in the HTE based on the schedule indicated in the
internship contract/ agreement;
1.6 Perform tasks and activities indicated in the Internship Plan;
1.7 Maintain confidentiality, when and where appropriate, during and after the
internship period of all data or trade secrets where such information is not
within the public domain and is indicated or understood to be confidential;
1.8 Adhere to the existing rules and regulations of the HTE, including the proper
use of tools, instruments, machines and equipment;
1.9 Complete the duration of his/ her internship: In case the intern will not be able
to finish the internship within the designated period, he/ she shall inform the
Students Internship Coordinator in writing of his/her intent and the reasons to
prematurely end his/ her internship three (3) working days before his/ her last
day of internship;
2.1 Design and implement the internship plan in partnership with the HEI;
2.2 Encourage student interns to develop their personality and professionalism
and to protect them from possible physical or moral danger;
2.3 Assign/ appoint a focal person responsible for the implementation of all
phases of the internship;
2.4 Orient the student interns on the standard rules and regulations of their
establishment;
2.5 Enter into an internship contract with the student interns;
2.6 Facilitate processing of the documents of student interns in coordination with
the HEI;
2.7 Provide practical training or work experiences in accordance with the agreed
internship plan and schedule of activities;
2.8 May provide necessary incentives to the student interns, such as free duty
meals, travel allowance and uniform, if applicable;
2.9 Ensure that student interns do not perform tasks and duties of a regular
teacher without supervision;
2.10 Provide supervised applied learning experiences for student interns in
accordance with the agreed Internship Plan and schedule of activities;
2.11 Develop feedback mechanism to the student intern and to the HEI;
2.12 Make available, accurate and current records and provide access to HEI
of such records while on internship;
2.13 Provides evaluation or other information on the student intern’s
performance as may be required by the HEI;
2.14 Notify and provide in at least 30 working days a written notice to the HEI of
a student intern’s breach of contract or misconduct in the internship premises
prior to HTE’s decision to suspend or terminate the contract; and
2.15 Issue certificate of completion for the student interns not later than two
weeks after the completion of internship.
This AGREEMENT shall take immediate effect upon signing by all Parties
concerned, commencing on February 13, 2023 (deployment period) and shall
end on May 17, 2023 provided that both PARTIES reserve their respective rights
to withdraw their participation in the AGREEMENT upon written notice.
