Step 4: Existing control measures in place

 PTW should be obtained from Almarai

 All hand-held tools inspected and free from any kind of hazard
 Proper fire extinguisher will be placed
 Proper housekeeping will be done at all times.

Description
Edge protection (parapet very low in height):

 Will ensure all work area near the edge are protected by barricade with MS pipe
 All employee briefed daily not to go near to the edge area.
 All work will commence on completion of edge protection of edge protection and inspection of contractor
safety staff.

Erection And Dismantling of Scaffolding for Utilities line installation and Tapping work H
 Scaffolding components shall be inspected by competent scaffolding personnel prior to scaffold erection to
ensure their quality
 Temporary covering of hot and cold line must be made before start the work.
 Make sure existing facilities should not damage during erection and distancing work
By provide the safe work techqines to carry out work through TBT
 Scaffold of different types such as cup coupler and tube coupler shall not be intermixed unless they are
compatible.
 Scaffold components shall be properly stored to prevent damage.
 Scaffold tubing shall be 48.3 mm (1.9 in) nominal outside diameter and other material must be checked by
scaffold inspector before start the erection.
 Scaffolding should be built by certified scaffold erector.
 Scaffold erector shall continuously wear a full-body harness with shock absorbing double lanyard while
erecting, altering or dismantling a scaffold.
 Scaffold erector shall properly anchor their lanyard whenever they are not protected by a guardrail system and
could fall more than 1.8 m (6 ft).
 Must be built on firm, level ground, base plate must be used and standards must be upright position
 Scaffold must be equipped with a guardrail system including top rails, mid rails, and toe boards
 Guardrail systems (consisting of top rails, mid rails and support uprights) shall be installed on all open sides
and ends of scaffold platforms
 The top edge height of top rails shall not be less than 0.95 m (38 in) and not more than 1.15 m (45 in) above
the walking/working surface of a platform or per site condition.
 Guardrail systems shall be completely installed before a scaffold platform or stair/ladder landing is used by
personnel other than the scaffold craftsmen.
 Planks shall not be placed on top rails or mid rail. Toe boards shall be installed along all edges of scaffold
platforms and Minimum 6” Toe Board must be installed. Scaffold platforms must be tightly planked with
scaffold plank grade material or equivalent
 Platforms shall be kept free of obstructions, unnecessary materials, projecting nails and other unnecessary
tripping hazards (including uneven decking).
 Adequate space for workers to safely pass shall be provided and maintained wherever materials are placed on
platforms.
 Platform units that have become slippery with oil, sand or any other substance shall be cleaned, or otherwise
removed and replaced, prior to continuing use.
 Ladders shall be installed in a safe and accessible location. Only one person shall climb the ladder at a time
and Three-point contacts shall be maintained while climbing ladders
 Scaffold materials must be place in a rack when transporting. Personnel must stay away and clear the area
prior for lifting of materials and working area must be covered with hard barricade.
 Damage materials must not be used and must be removed from site and Scaffold materials must not be
painted
 Only authorized personnel are allowed in the work area.
 Barricades with signage’s (Danger: Scaffolding Works in Progress) must be provided and posted to warn
other personnel of the ongoing activity.
 Sufficient clearness should be provided if any electrical cables there
 End caps should be provided in all exposed scaffold tubes in order to prevent from damage and injury caused
by exposed tubing.
 Tags should be displayed on scaffold with all necessary remarks for identify a safe use.
 Workers should be aware of heat, pneumatic, electrical and other sources that are existing in place where the
scaffold being erect nearby with, that awareness must be given by HSE officer of site.
 All involvers should have a clear idea of emergency evacuation route and signage’s and that should not be
obstructed

Arrangement regarding Heavy Equipment Parking:

 Ensure the crane is parked on level ground


 Ensure prior approval from Almarai concerned authority regarding crane positioning at the designated place
 Ensure all other sub-contractors are communicated and coordinated for lifting plan
 Crane placement is away from any excavation man holes and existing facilities
 Area is barricaded to avoid entry of un authorized vehicle

 Provision of flag man for signaling trailer truck for un loading
 Ensure outriggers are fully extended and pads are placed under the outriggers
 Ensure there is no overhead utility

Lifting Of Pipes, Material and Curbstone, Weight = 45 to 65 KG (CPP1 Roof Top Near New Canning Line H
Building) By using Crane (Work Procedure)

 Work to commence only on issuance of PTW.


 All safety instructions in PTW to be complied.
 Cranes must have a valid inspection sticker from an approved third party.
 In positioned lifts, outriggers shall be used with appropriate pads and mats. Booms shall not be swung without
outriggers being extended in accordance with the manufacturer’s
 Check the swing radius of the crane and ensure that it is unobstructed by any facility. Ensure that crane is
positioned correctly.
 Documents (Third party, operator license, Iqama) of crane and operator to be submitted to the authority.
 Crane lifts shall not be allowed at wind speeds above 32 km/h (20 mph) unless otherwise by the manufacturer.
 Do not position the crane outriggers near any drain line or any soft area otherwise there is a chance of sinking
of these outriggers resulting in crane tipping over.
 Tag-lines shall be used on all lifts, except when their use may create a greater hazard. Tag line must be used to
control the load and avoid hitting any plant facility like pipelines, equipment.
 Correct lifting angle must be used for lifting 60 degree is the best lifting angle.
 Correct capacity of slings and shackles must be used for lifting as per weight and shape of the load.
 All crane loads shall be rigged by a certified rigger.
 Use of PPE for all employees involved in task, such as hard helmet, reflect jacket, gloves, goggles, safety
shoes, face mask etc.
 Coordinate with concern authority, Safety, other contractors involved to the works.
 Restrict other Traffic movements, unauthorized entry to lifting / barricaded area.
 All combustible materials should be removed from nearby.
 All cab glass shall be safety glass and kept clean without obstructions or damage.
 The crane operator shall not leave the crane cab while the crane is attached to a load.
 Position (lower) hydraulic crane booms and secure hook while traveling.
 All lifting tackles must be in good working condition.
 All lifting tackles must be visually checked before use for any damage.
 There is no visual hydraulic leakage.
 Lifting will be done slowly and gradually according to lifting plan
 There is clear eye contact between rigger and operator
 Know the accurate weight of all loads and lifting gear prior to the lift.
 Site supervisor/Engineer / HSE officer to monitor the task.
 Avoid load swings over personnel or areas.
 Barricading of the working area using cones and warning tape.
 Use of proper sign boards and Lift area to be kept organized.
 Sufficient fire extinguisher to be positioned in lift area.
 Crane operator and rigger must be 3rd party certified.
 Wind speed and weather conditions to be considered and always follow load chart for lifting any load.
 Crane operator must not lift any load out of the load chart limit.
 Riggers shall inspect slings, and shackles prior to use.
 No one is allowed to be under the suspended load.
 Load must be secure on forklift and flagmen must be assigned to assist the forklift operator.

Installation Of Curbstone for Utility Pipe Support (Size=500 X 400 X 150 X 80 MM) (Approximately Weight H

Supply&Return,3inch) (First and Reuse Water,2inch&3inch) (Hot Water,2inch) (Compressed Air,3inch) &
Insulation cladding work

 Combustible materials (if present there) must be removed before start the work during installation of pipes and
support work and hot work area must be covered with fire blankets.
 Place welding machine on level ground, ensure all equipment is correctly tagged, inspect all power tools prior
to use, ensure all power leads are hanging from insulated hooks and are not lying on the ground, do not run
leads through scaffolds
 Ensure safe manual handling techniques are adopted by the workers
 Follow Watch Your Step technique and work area must be clear to avoid slip trip and fall before start the work
 Ensure right tool is used for the right job
 Provision of personal protective equipment during cladding and insulation work.
 Information, instruction and training regarding manual handling is given to employees
 Availability of fire extinguishers (CO2 and DCP) and trained fire watch

Tapping of New Utilities Lines (Steam,4inch) (Condensate,1-1/2) (Glycol Supply&Return,3inch) (First and
Reuse Water,3inch&3inch) (Hot Water,2inch) (Compressed Air,3inch) From Existing Main Lines
 Ensure must lines are isolated, lockout and tagged and properly drained before start cutting the lines and make
sure lines are isolated and drained to check through isolation valves and drain point and lockout must be done
by contractor along with tagged in which clearly identifies the location and purpose of tagged and clearly
mentioned the date and time and signed by the contractor responsible person.
 In case any leakage during work. immediately stop the work and informed related department and not resume
the work until line is clear.
 Job rotation procedure must be adopted to minimize the hot environment exposure and cold drinking water
must be available there all time.
 Ear muffs must be used by all team during work and other PPES must be used all time.
 Ensure proper work plat form is erected by competent personal and inspected for its safe use
 All work force will be informed and advised to never use (Existing facilities as work platform)

 Ensuring that overreaching and working /standing on guardrail is strictly prohibited
 Ensure cladding is removed carefully after the pipe is isolated, drained and de pressurized and is cool
 Ensure compliance with PPEs (mask and gloves and full sleeves shirt while removing insulation from pipes.
 All affected personal in area will be informed.
 Area barricaded and signage installed
 Ensure that all employees will be well aware of emergency response procedure and assembly point
 Ensure that tools and equipment used during tapping activity are pre inspected, safe
 Ensure compliance with mandatory PPEs (safety helmet, safety shoes, vest, glasses, mask) and secondary
PPEs (earplugs, earmuffs, face shield)
 Ensure close safety monitoring during tapping activity

 Availability of sufficient numbers of fire extinguishers is ensured at workplace

X-Ray (RT) For Welding Joints H


 Ensure area is isolated and barricaded
 No employee shall be permitted to use/operate ionizing radiation equipment without being adequately trained
and competent in the safe use and handling of ionizing radiation sources related to their job responsibilities.
 Employees shall be provided with and use the necessary personal monitoring and protection measures (e.g.,
personal protective equipment) while using/handling ionizing radiation equipment.
 Controlled areas (where high levels of radiation may exist during radiographic operations) shall be barricaded
and radiation warning signs shall be posted
 Access to controlled areas (high radiation areas) shall be restricted to authorized personnel only.
 Radiographic work shall immediately stop if unauthorized personnel enter a controlled area.
 Ensure adequate communication and coordination with all sub-contractors, project management team and
operation team are done
 Ensure trained and certified persons only doing RT
 Provision of stand by man to restrict un authorized person entry in restricted area
 Provision of close safety coverage during RT
Pressure Test for Pipes

 Ensure proper planning for pressure test has been made


 All other sub-contractors are pre notified regarding pressure test
 Only personnel involved with the pressure test shall be allowed near the system at any time during the test.
 Test pumps shall be attended at all times while in operation, unless isolated from the system.
 To prevent possible overpressure, test relief valves shall remain installed until the test liquid has been
completely drained.
 Relief valves shall be properly located in the system to be tested, including near the test pump. Relief valves
shall be tagged with the following information: the word “TEST”, the set pressure and a valid test date.
 Drains shall be installed at suitable locations to allow removal of the test liquid.
 Hoses connecting test pumps to manifolds shall be rated for the test pressure and shall be fitted with safety
chains or equivalent means strong enough to prevent whipping in case the coupling becomes disconnected.
 Pressure-testing manifolds, either for permanent or temporary usage, shall be tested separately from any other
piping prior to use with system testing. H
 Test areas shall be marked and barricaded. Warning signs shall be posted to alert approaching personnel (e.g.,
“Do Not Enter Testing in Progress”).
 During pressure tests, pressure rises shall be gradual and under control to allow time for material to strain and
for personnel to check for leaks.
 Upon test completion, the system shall be emptied in such a way as not to cause vacuum and collapse (e.g., a
liquid test medium shall be drained from the system slowly and with vents open).
 Systems shall not be depressurized by loosening bolts in a flange or by unscrewing fittings.
 Appropriate personal protective equipment (PPE) shall be worn by pressure test personnel. Additional PPE
shall be worn to protect against exposure to hot liquids above or to hazardous chemicals that may be used
during testing.
 Close supervision and safety coverage will be there at the time of pressure test
 Area is isolated and properly barricade and signage posted
 Pressure testing will be done by experienced work force only
Core Drill Work (wall coring) H

Pre-Operational Check:
 Ensure task (e.g., Drawings, instructions, specifications etc.) is clearly understood.
 Check that the accessories and attachments are properly fitted and that all guards and other safety devices are
in place and working properly.
 Before you inspect any parts, make sure the power is turned off and the lead unplugged from the power source.
 Check workspace and walkways to ensure no slip/trip hazards are present. Remove any flammable liquids or
combustible materials from the near proximity if present there.
 Identify ON/OFF switch and emergency stop button.
 Ensure all other employees are clear of the immediate work area.

Safe Operation:

 Do not use in close proximity to other workers.


 When you start up the machine, listen for unusual noises or vibrations.
 Use extreme caution when drilling through floors. Check for electrical conduit.
 Use a sufficient supply of water to ensure that hole is constantly being flushed of abrasive cuttings.
 While you're working, maintain a balanced position and keep a firm grip on the handles.
Always use two hands. Ensure you have a firm footing and are always ready to handle any reaction the tool
may make.
 Slowly lower the bit into the cut so that there is no skidding or movement of the drill bit. The entire
circumference of the core bit should penetrate the drilling surface before additional pressure is applied to the
handle.
 Do not stop the flow of water or the rotation of the bit as long as the bit is in the hole.
 Stop operation if the tool bounces around uncontrollably.
 Use extreme caution when drilling through floors. Provide protection of all personnel and material below the
area. Cores generally drop from the drill at completion of the hole.
 Drill stand must be properly secured. And Don't over-reach or work at an awkward angle.
 Do not touch the bit immediately after operation. It may be extremely hot.
 To stop, release the handle grip switch. Turn off and disconnect after use. Avoid breathing dust – wear suitable
dust mask or respirator in dusty areas.
 Be aware that this power tool is designed to create severe vibration. The hands, arms and legs will quickly tire.

Water Flushing of pipelines H

 For all pipes water flushing will be done


 Ensure tools and equipment used are pre inspected and are safe to use
 Ensure coordination with other subcontractor during job performance
 Ensure all end valves for flushing are closed and signage posted
 All Isolation valve will be identified / cross checked and LOTO Ensured.
 Ensure only authorized and competent person will do flushing
 Ensure drain point identified and drain will be done in only that drain point only
 Ensure provision of PPE while dealing with contaminated water
 Ensure proper arrangement of hoses and equipment to avoid trip and fall
 Ensure availability of sufficient numbers of fire extinguishers on work site
 Availability of first aid box will be made available at work site
 Availability of eye wash and safety shower near the work area will be ensured
 Ensure hoses and equipment are pre inspected and without any defect.
 Pressure rated hose will be used for filling pipe lines
 Stand by man will be assigned at drain point to ensure completion of safe drain process
Hot work (drilling, cutting, grinding and welding, for work & Use of compressed gas cylinder (Argon) for
welding, cutting and grinding
 All welders shall be certified for the materials and types of welding being performed and, when requested, shall
present their valid certificates.
 Combustible material around the work area shall be removed and/or protected against sparks, slag or heat using
fireproof material
 A qualified fire watch shall be assigned to any cutting, welding or burning operation. The fire watch shall
remain in the work area during these activities and for no less than 120 minutes after the hot work has finished.
 Cutting, welding and brazing equipment shall be turned off and compressed gas cylinder valves closed when
not in use.
 All cutting, welding equipment (including cylinders, hoses, and cables) shall be visually inspected by the user
prior to use and regularly maintained.
 Equipment with defects or damage (e.g., cracked hoses, cylinders with dents or broken regulator gauges) shall H
be immediately taken out of service for repair or removed from the job site and replaced.
 Compressed gas cylinders shall not be placed where they can become part of an electrical circuit (e.g.,
electrodes shall not come in contact with a cylinder). And NRV must be placed onto organ cylinders
 Personnel involved in cutting, welding operations, including helpers, shall use proper PPE. Respiratory
protection equipment shall be provided and used as needed.
 A welding sheet shall be worn during welding operations to protect the welder’s eyes and face against the
intense radiation (light/heat), flying particles, etc. A welding sheet or welding goggles (eyecup or cover spec
type) shall be worn by personnel performing cutting and brazing operations.
 Use guards (Flash screen with fire blanket) to confine the heat, spark and to protect the immovable fire hazards.
 Make suitable fire extinguishing (DCP & CO2) equipment immediately available in a state or readiness.

Use of power tools (angle grinder, drilling machine, cable drum, welding machine, etc...) H

 Appropriate personal protective equipment (PPE) shall be worn at all times when using hand tools and power
tools (e.g., hard hat, safety glasses/ goggles, hearing protection, safety shoes, gloves, face shield).
 Personnel shall not operate any tools unless they are appropriately trained in their selection, use, inspection and
storage. Training records shall be made available upon request.
 Tools constructed of good quality materials shall be used. Use of “homemade” tools is prohibited.
 Tools shall be kept clean at all times.
 Tools shall be inspected before and after use, as well as before storage
 Only properly insulated or nonconductive tools shall be used when work is performed on or near electrical
equipment. Insulation integrity shall be regularly inspected by an authorized/certified electrician.
 A record of tools issued, repaired or withdrawn from use shall be maintained
 Cords, cables, etc., shall be positioned where they are not likely to be damaged (e.g., run overhead) or shall be
otherwise protected from damage (e.g., running them inside impact resistant conduit or using abrasion resistant
supply leads)
 Residual current devices (RCDs), shall be used for all 110/220 V portable electric power tools
 Three pin plugs will be used for electrical equipment.

Use of chain block


 Will use chain block with suitable SWL and attached to installation supports.
 Only required employees will be made to stay near chain pulley working area.
 Work area will be barricade and signage provided.
 Experienced competent employees will be deployed and supervised. H
 All work will be closely supervised by supervisor and safety officer.
 No employees allowed standing below suspended load.
 No employees permitted to hold suspended load by hand, instead use guide ropes.

Manual Handling:
 Plan the move and measure access of the path.
 Ensure clear passage.
 Bring in professionals for large jobs.
 Use pallets and forklift where possible.
 Aim for a minimal distance carry and lift.

 First Aid facility will be made available on site to take care of unexpected first aid cases.
 Adequate supervision provided and emergency contacts will be provided with the workers in case any
incidents/Accidents.
