legal age, Filipino, single/married and both presently residing at 218 Don Fabian St., Brgy,
Commonwealth Quezon City, both after being duly sworn to in accordance with law, depose and

1. We personally know HARLENE MUŇOS GUIWANON, who was born on May
20, 2000 at Caloocan City;

2. We know for a fact that HARLENE MUŇOS GUIWANON lives in and a resident
of Block 31 Lot 6 Dama De Noche St. Phase 2 Caloocan City, that she has lived there
since childhood with her siblings and parents;

3. We are not in any way related to foregoing going person within the fourth civil
degree either by affinity or consanguinity, now do we have any pecuniary interest in
this matter whatsoever;

4. We are executing this affidavit to attest to the truthfulness of the foregoing statements
and for whatever legal purpose or purpose this may serve under the law.
