III. PRESENTING PROBLEM


Lhance Darell C. Piga is a 1 year old and 7 months toddler experiencing developmental
delays due to some complication from his birth. The major concern right now is to help
Lhance cope up with his Cognitive Delays to cover the developmental gaps.

IV. BACKGROUND/HISTORY
a) Client
Lhance Darell Corpuz Piga was born on the 7 th day of May 2021 at Baggao Rural
Health Unit and Birthing Center and was on labour for 6 and a half hours. Due to the
difficulty in the child’s birth, the child have swallowed amniotic fluid and was born
with a weak vital signs, later on admitted at the Baggao District Hospital and was
confined for 5 hours, later transferred to the Aparri Provincial Hospital and confined
for 2 weeks. In his first 2 days at the hospital the findings states that the child is
experiencing Developmental Delays. After the 12 days of recovery he was then
discharged from the hospital the Doctor advised that Lhance should undergo regular
check-ups for the possible reoccurrence of his seizure attacks as well as the concern
to his development. He was then consulted at Dr .Cherry Lou Antonio’s Clinic and it
further explains Lhance condition and the findings states that Lhance is experiencing
Generalized Epilepsy and developmental delays, in the present time the Child has
undergone medication for 1 year 1 month from now having regular check-ups in
every 2 months. His seizures now oft from reoccurring and improvement is evident,
the concern right now is the progress on his Development.
b) Family
Mr.Algie Piga and Ms. Maybel Corpuz are common law, living under the roof of
Mrs.Erlinda Piga who is already widowed for 4 years now. Mr. Algie is the youngest
of the 3 siblings, he is a graduate of Bachelor of Industrial Technology Major in
Electronics but currently unemployed he is now working as a farmer with 5,000 pesos
as his monthly income covering the every 2 months check-up of Lhance worth 3,500
pesos. Ms. Maybel Corpuz is now continuing her studies at Cagayan State University
Taking up Bachelor of Industrial Technology Major in Food Technology, as the F To
F classes opens Lhance have to switch from breast feeding to bottle feeding. Lhance
was always left to his father and grandmother’s care. Meanwhile Mrs Erlinda is a
senior citizen unavailable to work for a living now but when it comes to the practice
of authority in the house Maternal authority is prominent.
c) Environment
The Family lives in a wood and concrete type of house with an access of electricity,
clean water supply, owned a proper toilet with a clean and orderly surroundings, the
house is large enough to accommodate the four of them with 3 bedrooms. They have
a good relationship with their neighbours specially Mrs Erlinda who is assigned as
alms collector of their respective zone, Mr Algie is working for his cousin Mr.Gamata
as one of the caretakers of their farm not far away from their home. The house is
located near the main road about 3 meters away.

V. Assessment
Lhance Darell a 1 year old and 7 months old child is continually showing improvements,
his seizures oft occur and his Sensorimotor Delays is still on process of improving,
changes is evident because of the regular medication and simple massage therapy at
home. As of Piaget’s Cognitive Development Theory Stage 1 is the Sensorimotor Stage
from birth to Age 2, states that a child should be able to develop reflexes, habits, hand-
eye coordination or simply the looking, hearing, mouthing, and grasping which was not
yet developed with Lhance, he still struggle to move his hands to grasp the things handed
to him he simply develop the “close-open” pattern with his hands as well as just to swing
his right leg. He was now also able hold the strength to lift his head but his hearing senses
aren’t that developed yet, he barely produce sound from laughing and crying but in the
positive side Lhance is continuously showing signs of improvement not that fast but
process is taking smoothly. As of the present time Lhance needs assistance in further
improving his sensorimotor skills to cope up the gap of his developmental delays. The
Strength Perspective can be in use to improve Lhance’s situation stressing the point that
this perspective is focused unto the ability or capacity of the client to attain the goal it
could be of use to look forward to Lhance’s improvement as the child is thriving and
positively responding to the Physical therapies there is a great possibility that overtime
his sensorimotor skills would develop and would be incline with his age as the Cognitive
Development Theory of Piaget states.

VI. Intervention Plan


In the helping process Lhance should be able to receive:
a. 4 times a month Massage Therapy (physical therapy)

VII. Recommendation
• The Social Work Student is still not able to produce financial assistance for the child
needs in medication concerning the reoccurrence of his seizures which is one of the great
factor contributing to the damages to his brain which causes his Developmental Delays,
this concern must be addressed too as soon as possible. The Physical Therapy should be
continued until the developmental gaps are coped up.

Orienting Theories
Jean Piaget’s Cognitive Development proposes that a child would acquire a certain set of skills
or strategies it would move up a step to the next stage.it was a progressive reorganization of
mental processes as a result of biological maturation and environmental experiences. Cognitive
Development involves changes in cognitive process and abilities. In Piaget’s view, early
cognitive development involves processes based upon actions and later process to changes in
mental operation.
In Piaget’s Cognitive Theory contains 4 stages particularly the sensorimotor stage, the
preoperational stage, the concrete operational stage, the formal operational stage, with those
stages Lhance failed to pass the first stage because his sensorimotor skills aren’t fully developed
due to his developmental delays, thus this theory can be of use to as the guide for Lhance’s
development.

Practice Perspective
The Strength Perspective focuses on capacity and capability of the client to positively respond
amidst the helping process. It helps client to seek and build the positive qualities within them
basically looking over to their strength rather than their weakness believing in their talents as
well as the resources available in their support networks, such as their family, friends and wider
community.
In accordance to Lhance Darell’s situation amidst of his struggle caused by his developmental
delays improvement is still evident through the efforts of his support network to help him cope
the developmental gaps. Lhance is positively responding to every Physical Therapy Session as
well as to the weekly kamustahan which his responding senses where tested by teaching him
basic skills to improve. And with all those intervention Lhance is responding positively, thus
Strength Perspective compliments and is fit with Lhance situation to be applied for the focus is
unto Lance’s capability to continuously improve rather than focusing to his inability the aim is to
look forward to his further improvement.

Practice Theory
The Social Learning Theory by the Psychologist Albert Bandura suggest that social behaviour is
learned by observing, modelling and imitating the behaviour of others. This theory has often
been called a bridge between behaviourist and cognitive learning theories because it
encompasses attention, memory and motivation.
In connection to Lhance current Situation he is still in the start of learning process by simply
imitating movements, stating that age 1 marks the beginning of true imitation with intent.
Lhance’s ability to respond to his surrounding can be poorly rated at some point, his gross motor
skills are still on the process of improving because of his developmental delays. But on the other
hand this gaps in his skills would eventually be coped up by simply teaching him, practicing or
modelling acts he could easily imitate for instances is showing him simple gestures like clapping,
waiving and reaching out his hands, it could also help if he would be exposed to his fellow
toddlers as he would be observing and would come to imitate his playmates behaviour. Thus
Social Learning theory can be of help to into developing his skills by simply learning or
imitating.

Practice Models
Task-centered practice focuses on actionable solutions to specific problems. It is easy and
measureable. It identifies the client’s issue, outline goals, creates and execute a task-centered
plan, and evaluate the result.
With Lhance current situation, the problem or issue is easily addressed and have been given
focus rooting from the factor that is causing his developmental delays which was his Generalized
Epilepsy causing the reoccurrence of his seizures that basically causing damage to his brain
resulting to his developmental delays particularly with gross motor skills. In the present time
Lhance is undergoing medication especially for the reoccurrence of his seizures and looking with
the positive result his seizures oft occurred only leaving with his motor skills delayed
development. The goal is to help Lhance cope up the developmental gaps hence the intervention
plan should be carried out until the addressed problem has been solved, only focusing to solve
his developmental delays.
