AFFIDAVIT OF WITNESS

I, Jihyo y Alonso, 26 years old , Single, resident of No.132 Kalaw Street Brgy. Paykek, Bolinsak,
Kapangan, after having sworn in accordance with law, do hereby depose and state THAT:

01. I was present and personally saw that accused, Daniel y Menario, committed the crime of
Violation Of Republic Act No.9165 or The comprehensive Dangerous Drugs acts. Of 2002 and
Reckless Imprudence Resulting To Homicide against the victim Sana kim;

02. On or About 20:00H of 28th of June, 2022 While We Stayed In “Bypass” at Bolinsak,
kapangan Aong The kalaw street, I observed That, Mr. Daniel y Menario, He was Drunk Since he
came from Br Hauz At the Front of Bypass, while Miss. Sana Kim is Sitting In the sidewalk while
Selling (Balot and Penoy) ;

03. While Mister Daniel was Walking, He Crossed the bypass, because There was some one
waiting him on the side, and I saw him take something in his belt bag, I came a little closer to see
what it was, because I’m a bit curios, so I came closer, then I saw a packed of (Shabu) illegal drugs,
and marijuana that he actually uses and sells to the person waiting him, Mr. Menario, Notice me when
I’m about to call the police so he run and he took his motorcycle that was parked and accidentally Hit
Miss. Sana Kim and Accidentally Kill her.

04. Inn Witness Thereof ; I hereby set our Hand this 1st day of July 2022, at Municipal Court
Of Bolinsak, Kapangan.

I executed this affidavit to attest the truthfulness of the foregoing facts and to support the filing of
Criminal Cases against Daniel y Menario for violations of Republic Act No.9165 or The comprehensive
Dangerous Drugs acts. Of 2002 and Reckless Imprudence Resulting To Homicide.
