The above named is a known patient of the cardiopulmonary unit of this facility since 2021. She is being
managed as a case of severe bronchial asthma and has a previous history of chronic peptic ulcer.

She has been placed on medication and constant monitoring and was strongly advised to avoid any
environment or activities that would trigger the symptoms.

In line with the above, kindly grant her exit to camp as she is unfit to participate in camp vigorous
activities.

Kindly accord her all the necessary assistance needed.
